Özet
Romatoid Artrit (RA) etyolojisi bilinmeyen, simetrik eroziv sinovitle karakterize, bazı extraartiküler tutulumlar yapan, otoimmün bir hastalıktır. Sarkopeni, kas kuvveti ve kas protein kitlesi kaybına ek olarak fonksiyonel kalitenin azalmasını içerir. Sarkopeninin en yaygın nedeni yaşlılıktır. Son yıllarda yapılan çalışmalar kronik inflamatuar hastalıkların sarkopeniye neden olduğunu göstermişlerdir.Bizim bu çalışmayı yapmaktaki amacımız tüm dünyada sık görülen kronik inflamatuvar bir hastalık olan RA ile sarkopeni arasındaki ilişkiyi değerlendirmekti. Çünkü literatürü incelediğimizde bu konuyla ilgili çok az sayıda çalışma vardı.American College of Rheumatology (ACR) 1987 yılında yenilediği RA klasifikasyon kriterlerine göre belirlenmiş, hastalık süresi en az iki yıl olan 30 RA'lı bayan hasta ile 30 mekanik bel ağrılı bayan kontrol grubu olarak çalışmaya alındı. Olgularda DXA ile vücut kompozisyonuna bakılarak RA ile sarkopeni arasındaki ilişki araştırıldı. Ayrıca çalışmada yer alan RA'lı hastaların CRP, ESR düzeylerine bakıldı ve DAS28 skorları hesaplandı. Sarkopeni kadın ve erkeklerde farklılık gösterdiğinden çalışmadaki gruplar bayan hastalardan oluşturuldu.İskelet kas kitle indexi (SMI) RA'lı hasta grubunda kontrollerle karşılaştırıldığında daha düşük bulundu: sırasıyla 5,83±0,807 ve 30±1,64 (p=0.022). Sarkopeni (kadında SMI?5,75 kg/m2) RA'lı grupta daha fazla bulundu ve fark istatistiksel olarak anlamlıydı (p=0,004). Sarkopeni, BMI'ye göre normal ve fazla kilolu olan RA'lılarda, obez olanlara göre daha fazlaydı. RA'lı grupta sarkopeniyle DAS28 arasında ilişki saptanmazken (p=0,530), CRP yüksek olanlarda sarkopeni anlamlı olarak fazla bulunmuştur (p=0,013). RA'lı grupta ilaç kullanımı ile sarkopeni arasında ilişki bulunamamıştır (p>0,05).Sonuç olarak çalışmamız; RA'lı hastalarda SMI'nin azaldığını, sarkopeni riskinin arttığını, obez olmayan hastalarda riskin daha fazla olduğunu ortaya koymaktadır.Anahtar Kelimeler: Romatoid Artrit, Sarkopeni, DXA
Rheumatoid arthritis is an autoimmune disease that is characterized by symmetric erosive synovitis with some extra-articular involvements and its etiology is unknown. Sarcopenia includes diminution in functional quality in addition to loss of muscle strength and muscle protein mass. Old age is the most common reason of sarcopenia. Recent studies showed that chronic inflammatory diseases cause sarcopenia.In this study, we aimed to asses relationship between sarcopenia and rheumatoid arthritis which was a commonly seen chronic inflammatory disease in whole world. Because, there was a few studies related with this issue in the literature.We recruited 30 women with minimum two year duration of rheumatoid arthritis (RA) and 30 women with mechanical low back pain as controls in out study according to RA classification criteria of American College of Rheumatology (ACR) which was reestablished in 1987. In these patients, relationship between RA and sarcopenia was assessed by evaluating DXA and body compositions. C-Reactive Protein (CRP), Erythrocyte Sedimentation Rate (ESR) levels were determined and DAS28 scores were estimated in the patients with RA. We compromised the groups from female patients because sarcopenia differs between male and female individuals.Skeleton muscle index (SMI) was found lower in RA group when compared to controls: SMI was 5.83±0.807 and 30.0±1.64, respectively (p=.022). More sarcopenia was found in RA group and the difference is statistically significant (p=.004). Sarcopenia was more common in normal and overweight patient with RA than obese patients with RA. While no relation was found between sarcopenia and DAS28 score in RA group (p=.530), sarcopenia was found more common in patients with elevated CRP (p=.013). We didn?t found any relation between drug use and sarcopenia in RA group (p>.05).Consequently, our study established that, in patients with RA, SMI decreases and risk of sarcopenia increases and risk is higher in non-obese patients.Key words: Rheumatoid Arthritis, Sarcopenia, DEXA
