Özet
Amaç. Bu araştırma tip 2 diabetes mellitus (DM) olgularında odyometri ve işitsel beyin sapı yanıtı testlerinin klinik önemini incelemek amacıyla yapıldı. Yöntem. Tip 2 DM tanısı konan 33’ü kadın, 17’si erkek olmak üzere toplam 50 hasta ile aynı yaş ve cinsiyetle eşleştirilmiş 50 kontrol grubu bireye kulak burun boğaz muayenesi yapıldı; saf ses odyometri, yüksek frekans odyometrisi ve işitsel beyin sapı yanıtları testleri uygulandı. Çalışmada elde edilen verilerin istatiksel değerlendirmesi Fisher kesin ki-kare, ki-kare ve iki ortalama arasındaki farkın önemlilik testleri ile yapıldı. Bulgular. Diabetik olgular ile kontrol grubunun işitsel beyin sapı yanıtları karşılaştırıldığında I. dalga, III. dalga mutlak latanslarında ve I-III interpeak latansta anlamlı bir fark yok iken V. dalga mutlak latansında ve III-V ile I-V interpeak latanslarda anlamlı uzama saptandı. Sonuçlar. Periferik iletim zamanını gösteren I. dalga mutlak latansının normal, santral iletim zamanını gösteren V. dalga mutlak latans ve III-V ile I-V interpeak latanslarında uzama olmasının beyin sapı üst seviyelerinde diabetik nöropati etkisinin daha belirgin olduğunu göstermiştir. Diabetli olgularda işitme sinirinin iletim hızının santralde daha fazla etkilendiği görülmüştür.
Aim. The aim of this study was to evaluate clinical value of audiometry and auditory brainstem responses (ABR) in type 2 diabetes mellitus (DM) patients. Methods. Fifty patients with type 2 DM (33 females, 17 males) were included in the study with 50 age- and sex- matched controls. All patients and controls underwent ear-nose-throat examination. Pure tone audiometry, high frequency audiometry and ABR were performed. Statistical analysis was performed with Fisher’s exact, χ2, and variance analysis tests. Results. ABR of diabetic patients and control group were compared. There was no significant difference between I. wave, III wave absolute latency and I-III interpeak latency, significant elongation was determined in V. wave absolute latency and III –V and interpeak latency. Conclusions. Normal I. wave absolute latency, which shows peripheric conduction time and elongated V. wave absolute latency and II-V and I-V interpeak latency, which show central conduction time, point that diabetic neuropathy affection the upper part of the brain stem more obviously. İn diabetic patients conduction velocity of auditory nerve is especially affected centrally than the peripheric.
