Despite ten units of blood transfusion within less than the first 24 hours of admission, hemodynamic state was not stabilized. The patient was underwent surgical exploration and a subtotal ileocolectomy was carried out with ileostomy and colonic mucous fistula. Three months later, his ileostomy was closed and no evidence of disease recurrence was observed during the past three years. Massive rectal bleeding from Crohn's disease is a very rare entity especially in children.
