Diabetik retinopatinin erken tanısında kontrast sensitivitenin yeri ve önemi

Abstract

Amaç: Bu çalışmanın amacı, erken fonksiyonel değişikliklerin tespit edilmesinde Cambridge Low Contrast Grating testi ve glikozile hemoglobinin (HbA1c) yeri ve önemini belirlemektir. Gereç ve Yöntem: Bu çalışmaya, hasta grubu olarak 46 tane diabetik hasta alındı. Hasta grubundaki olgular retinopatisi olup olmamasına göre 2 gruba ayrıldı. Retinopatisi olmayan 24 DM'lu olgu Grup 1, background retinopatisi olan 22 DM'lu olgu Grup 2 olarak tanımlandı. Kontrol grubu olarak refraksiyon dışında oküler patolojisi olmayan 20 olgu alındı. Bu gruplar kontrast sensitivite testi sonuçlan ve HbA1c düzeyleri yönünden karşılaştırıldı. Bulgular: Kontrast sensitivite testi sonuçlan grup 1'de 135±5.98, grup 2'de 92.85±4.46 ve kontrol grubunda 155±5.00 idi. Çalışma grubu sonuçları kontrol grubuna sonuçlan ile karşılaştırıldığında istatistiksel olarak anlamlı şekilde düşüktü (F=33.58, p<0.05). HbA1c sonuçları ise grup 1'de %9.79±0.81, grup 2'de %10.55±0.80 ve kontrol grubunda %4.81±0.09 idi. Çalışma ve kontrol grubu sonuçları arasındaki fark istatistiksel olarak anlamlı idi (KW=34.81, p<0.05). Sonuç: DM'un erken döneminde retinada fonksiyonel hasarın olabileceğine ve hasarın tespitinde kontrast sensitivite testinin önemli yeri olduğuna karar verdik. Retinopatinin tanısı ve takibinde bu testlere ek olarak HbA1c düzeylerinin izlenmesi gerektiğini de düşündük.

Purpose: The aim of this study is determining the value and importance of Cambridge Low Contrast Grating Test and HbA1c levels in detecting early functional changes. Materials and method: Forty six diabetic patients participated in this study as the patient group. The subjects in the patient group were divided into two groups in respect of having retinopathy or not. Twenty four patients without retinopathy were defined as Group 1 and 22 patients with back-ground retinopathy were defined as Group 2. A control group of 20 subjects who had not an ocular disease except a refraction deficit participated in the study. These groups were compared in that contrast sensitivity test results and glycolysed hemoglobin levels. Results: The results of contrast sensitivity test were 135&plusmn;5.98 in Group 1, 92.85&plusmn;4.46 in Group 2 and 155&plusmn;5.00 in control group. There was a statistically significant decrease in the results of patient groups compared with control group. The results of HbA1c levels were 9.79&plusmn;0.81 in group 1,10.55&plusmn;0.80 in group 2 and 4.81&plusmn;0.09 in control group. The difference between study and control group results was statistically significant. Conclusion: We concluded that the retinal functional deficit may occur in early stages of DM and contrast sensitivity test has an important value in detecting this functional deficit. We also thought that the evaluation of HbA1c levels must be added to these test in diagnosis and management of retinopathy.
