Trabekülektomili olgularda katarakt ekstraksiyonunun göz içi basıncına etkisi

Abstract

AMAÇ: Bu çalışmada daha önce trabekülektomi ameliyatı geçirmiş ve fonksiyonel blebi olan glokomlu hastalarda, katarakt ekstraksiyonunun göz içi basıncına(GİB) etkisi araştırıldı GEREÇ VE YÖNTEM: Retrospektif olarak yapılan bu çalışmaya fonksiyonel blebi olan ve ekstrakapsüler katarakt ekstraksiyonu (EKKE) ve arka kamara göz içi lens(AKGİL) implantasyonu yapılan 12 glokomlu (13 göz) hasta alındı. Tüm hastalar EKKE+AKGİL implantasyonu sonrasında en az 9 ay takip edildi. Hastaların EKKE+AKGİL implantasyonu sonrası ortalama GİB, 1. hafta, 1.,3.,6. ve 9. aylarda ölçülerek preoperatif ortalama GİB ile karşılaştırıldı. Ayrıca bu süre içinde kullanılan ortalama antiglokomatöz ilaç sayısı da karşılaştırıldı. BULGULAR: EKKE+AKGİL öncesi ortalama GİB 17.23 ± 7.10 mmHg iken, EKKE+AKGİL sonrası 1. hafta, 1., 3., 6. ve 9. aylardaki GİB değerleri sırası ile 17.70 ± 3.73 mmHg, 17.31 ± 4.48 mmHg, 17.92 ± 3.45 mmHg, 17.31 ± 1.97 mmHg ve 17.85 ± 1.77 mmHg idi. EKKE+AKGİL öncesi ile sonrası tüm zamanlarda, ortalama GİB'ları arasında istatistiksel olarak anlamlı fark yoktu(p>0.05). Ayrıca preoperatif ve postoperatif antiglokomatöz ilaç sayısı bakımından da istatistiksel olarak anlamlı fark saptanmadı (p>0.05). Postoperatif takip süresince tüm hastalarda fonksiyonel blebin devam ettiği saptandı. SONUÇ: Fonksiyonel blebi olan glokomlu hastalarda EKKE+AKGİL implantasyonunun, takip süresi boyunca GİB'na önemli bir etkisinin olmadığı düşünüldü.

PURPOSE: In this study, the effect of cataract extraction on intraocular pressure (10P) was studied in glaucoma patients who had previously undergone trabeculectomy. MATERIAL AND METHOD: Twelve glaucoma patients (13 eyes) with functioning filtering blebs had extracapsular cataract extraction and intraocular lens (IOL) implantation in this retrospective study. After extracapsular cataract extraction (ECCE) and IOL implantation, all patients were followed for at least 9 months. After ECCE and IOL implantation, the mean IOP of the patients were measured at 1 week, 1,3,6 and 9 months and were compared with preoperative mean IOP. Also, the mean number of ocular hypotensive drugs were compared at 1 week, 1,3,6 and 9 months. RESULTS: Before ECCE and IOL implantation the mean IOP was 17.23 ± 7.10 mmHg, after ECCE and IOL implantation the mean IOP values at 1 week, 1, 3, 6 and 9 months were 17.70 ± 3.73 mmHg, 17.31 ± 4.48 mmHg, 17.92 ± 3.45 mmHg, 17.31 ± 1.97 mmHg and 17.85 ± 1.77 mmHg, respectively and there was no statistically significant difference between the mean IOP (p>0.05). In addition there were no statistically significant difference, between the number of ocular hypotensive drugs used pre and postoperatively (p>0.05). During the postoperative follow-up period, all patients had functioning filtering blebs. CONCLUSION: In the patients with functioning blebs, during the follow-up period there was no significant effect of ECCE and IOL implantation on IOP.
