Taze dışkı örneklerinden Entamoeba türlerinin üretilmesi
Abstract
Entamoeba histolytica üzerinde en çok araştırma yapılan parazitlerden biridir. Buna karşın yine de hakkında bilmediğimiz pek çok nokta bulunmaktadır. In vitro olarak aksenik kültürü de yapılmış olan bu parazit konusundaki en büyük iki sorundan biri rutin tanısında karşılaşılan zorluklar, diğeri de hangi koşullarda dokuları istila ettiğinin net olarak anlaşılamamış olmasıdır. Yurdumuzda ise kültür çalışmaları kısıtlıdır. Çünkü her tür kültür çalışmalarında olduğu gibi Entamoeba histolytica kültüründe de bir alt yapının, daha doğrusu ekibin bulunması ön koşuldur. Halbuki buna sahip bilim/anabilim dalımızın sayısı çok sınırlıdır. Çalışmamızda, basit ve modifiye difazik bir besiyerini kullanarak laboratuvar koşullarımızda Entamoeba histolytica ve E. coli kültürlerini gerçekleştirip gerçekleştiremiyeceğimizi araştırmayı amaçladık. Besiyeri, 1:10 oranında sulandırılmış Ringer eriyiği içine kırılmış dört yumurta süspansiyonunun süzüldükten sonra koagüle edilmesi ve üzerine tamponlu serum fizyolojik eklenmesiyle hazırlandı. Ekimden önce besiyerine 800/mL ünite penisilin ve kırılmış, steril pirinç granülleri eklendi. Dışkı örnekleri, ağzı pamukla kaplı kültür tüpleri içindeki besiyerlerine, bakteriyolojik lup öze kullanılarak direkt olarak ekildi. Tüpler 37 "C deki etüvde inkübe edildi ve 48-72 saatte bir kontrol edildi, üreme olmasa da iki kez pasaj yapıldı; iki pasajda da üreme görülmeyince sonuç negatif kabul edildi, ikisi hastane olgularından diğerleri de ilköğretim okulu öğrencilerinden sağlanan toplam 68 dışkı örneğinden 21'inde (%30.8) üreme görüldü, bunlardan bir kısmı yedi pasaj devam ettirildi. Altmış sekiz örneğin direkt incelenmesinde 6'sında sadece E. histolytica kistleri, 33'ünde sadece K. coli kistleri ve 29'unda da hem E, histolytica hem de E. coli kistleri saptandı. Üreme görülen 21 kültürün ikisinde sadece E. histolytica, sekizinde E. coli ve 11 'inde de iki parazitin kistleri bir arada görülmüştü. Enianweha histolytica is one of the most investigated protozoon parasites of man. However, in spite of these studies still there are so many things about this protozoon which we cannot explain. One of these unsolved problems in regard to E. histolytica is the difficulty which one encounters in the definite diagnosis of entamebiasis. The second one is the pathogenicity of E. histolytica. Indeed it is almost impossible to determine the conditions which bring about the invasion of intestinal tissues by this amoebae. E. histolytica can be cultivated axenically in Diamond's medium. It can also be cultured in several xenic media. In our country, however, the cultivation studies of this parasite are relatively rare. We have tried to culture both E. histolytica and E. coli from their cysts which we found in fresh stool specimens during our survey of intestinal parasites in primary and secondary school students. We have tried to cultivate both E. histolytica and E. coli in a very simple medium consisting of four whole chicken eggs suspended in 50 ml of diluted Ringer's solution (1:10). This suspension was filtered, distributed in 5 ml amounts into sterile culture tubes and coagulated in a slanted position in an autoclave with the lid half open at 70 to 80° C for one hour. This was repeated twice on consecutive days. Then 5 ml of sterile buffered saline containing 800 units of penicillin/ml was added into each tube under sterile conditions. Before inoculation of the stool specimens, a few grains of sterile rice were added to each culture tube. Two tubes were used for each specimen and inoculations were done directly from the container to the culture tube using a bacteriological loop. The culture tubes were incubated at 37" C and controlled at 48 and 72 hours. Two subcultures were made from each tube even if growth of the amoeba was not detectable. After these two subcultures the culture was considered to be negative if no growth was observed. A total of 68 stool specimens were cultured in this medium and we have found trophozoites of amoebae in 21 (30.8%). Out of these 68 stool specimens, 6 contained only E. histolytica cysts; 33, only E. coli cysts; and 29, both types of cysts. Two of the 21 trophozoite positive cultures had only E. histolytica cysts; 8, only E. coli cysts; and 11, the cysts of both amoebae.
