Klinik örneklerden soyutlanan Enterococcus faecalis ve Enterococcus faecium kökenlerinin çeşitli antibiyotiklere direnci
Özet
Bu çalışmanın amacı, klinik örneklerden soyutlanan enterokok kökenlerinin çeşitli antimikrobiyallere direncini saptamak idi. İncelenen 120 kökenin 94 (% 78)'ü Enterococcus faecalis, 26 (% 22)'sı E. faecium olarak tanımlandı. Kökenlerin ampisilin, penisilin, vankomisin, teikoplanin, siprofloksasine karşı duyarlılığı NCCLS önerileri doğrultusunda mikrodilüsyon yöntemi kullanarak araştırıldı. Gentamisin ve streptomisine yüksek-düzey direnci agar tarama yöntemi ile araştırıldı. Enterococcus faecalis kökenlerinde direnç oranları, penisilin ve ampisilin için % 11, siprofloksasin için % 14 olarak; E. faecium kökenlerinde ise, penisilin ve ampisilin için sırasıyla % 77 ve % 65 ve siprofloksasin için % 38 olarak bulundu. Gentamisin ve streptomisin yüksek-düzey direnç oranları, E. faecalis kökenleri için sırasıyla, % 5 ve % 16 olarak, E. faecium kökenleri için % 23 olarak saptandı. Dört E. faecalis (% 4) ve dört E. faecium (% 15) kökeninin her iki aminoglikozide dirençli olduğu bulundu. Kökenlerin hepsinin vankomisin ve teikoplanine duyarlı olduğu saptandı. Enterokokların antibiyotik duyarlılık paternlerinin periyodik olarak izlenmesinin, bu bakterilerin neden olduğu infeksiyonların ampirik tedavisi için gerekli olduğu düşünülmektedir. The purpose of this study was to determine the antibiotic susceptibility of Enterococcus strains isolated from clinical specimens. Totally 120 strains were identified, Enterococcus faecalis (94: 78 %) and E. faecium (26: 22 %). The isolates were tested for susceptibility to ampicillin, penicillin, vancomycin, teicoplanine, and ciprofloxacin by using microdilution test according to NCCLS standards. High-level resistance to gentamicin and streptomycin was investigated by standard agar screening method. The rates of resistantance of E. faecalis strains were found as 11 % for penicillin and ampicillin, and % 14 for ciprofloxacin. Enterococcus faecium strains showed higher resistance rates for penicillin, ampicillin (77 % and 65 %, respectively) and ciprofloxacin (38 %) than E. faecalis strains . The rates of high level resistance of gentamicin and streptomycin were detected as 5 % and % 16 for E. faecalis strains, respectively, and 23 % for E. faecium strains. Four strains of E. faecalis (4 % ) and four strains of E. faecium (15 %) were found to be high-level resistant to both aminoglycosides. All of the strains were susceptible to vancomycin and teicoplanine. Periodic evaluation of the antibiotic susceptibility patterns of enterococci is necessary for the empirical treatment of infections due to these bacteria.
