dc.description.abstract | Amaç: Kronik progresif eksternal oftalmopleji tanısı konulan olgularımızın klinik özelliklerini ve tedavi yaklaşımımızı retrospektif olarak değerlendirmek. Gereç ve Yöntem: Bu çalışma 1992-2002 yılları arasında klinik bulgularına göre kronik progresif eksternal oftalmopleji tanısı konulan 18 olguyu kapsamaktadır. Olgular genel bir oftalmolojik muayeneden sonra, göz kapağı ve hareketleri açısından değerlendirildi. Ptozisin görme aksını kapattığı ve günlük yaşamı etkilediği olgulara, ptozis miktarı ve levator fonksiyonuna göre farklı prosedürler uygulandı. Bulgular: Çalışmaya yaş ortalaması 33.7±18.3 yıl olan, ll'i erkek 7'si kadın 18 olgu alındı. 12 olgunun 20 göz kapağına ptozis cerrahisi, levator ve frontal adele fonksiyonu olmayan bir olguya da görmeyi sağlamak amacı ile bilateral cilt-adele eksizyonu uygulandı. Ortalama takip süresi 22.2 ay idi (en az 6 ay, en çok 10 yıl). Ameliyat sonrası takip süresi içinde 6 olguda erken devrede ortalama 3 ay süren oküler yüzey irritasyon bulguları, bir olguda kornea ülseri ve bir olguda da sentetik materyalin alındaki tespit yerinden ekspojum gözlendi. Sonuç: Kronik progresif eksternal oftalmoplejili olgularda sosyal aktivite ve günlük yaşantıyı etkileyen ileri derece ptozis oluştuğunda cerrahi başarılı bir şekilde uygulanmaktadır. Ancak göz hareketlerinin sınırlı veya olmayışından dolayı oluşabilecek komplikasyonlardan kaçınmak amacıyla, kapak aralığının sınırlı miktarda kaldırılması ve erken postoperatif dönemde olguların yakından takip edilmesi gerekmektedir. Bu nedenle görme aksı kapanmayan olgularında ptozis tamiri yapılmadan izlenmelerinin uygun olacağı düşünülmektedir. | en_US |
dc.description.abstract | Purpose: We evaluated the clinical features and the therapeutic approach in cases with chronic progressive external ophthalmoplegia. Material and Method: We evaluated the records of 18 cases who were diagnosed as chronic progressive external ophthalmoplegia according to the clinical findings. We examined the eyelids and ocular motility of the patients following routine ophthalmic examination. In patients with severe ptosis covering the visual axis and affecting the daily life we applied different procedures according to the amount of ptosis and levator function. Findings: The study group consisted of 11 male,7 female patients with a mean age of 33.7+18.3 years. We performed ptosis surgery to 20 eyelids of 12 cases and one patient with poor levator and frontal muscle function underwent bilateral skin-muscle excision in order to provide vision. Mean follow-up period was 22.2 months (6 months-10 years). Following the operation we observed ocular surface irritation findings for a period of 3 months, in 6 cases, corneal ulcus in one and protrusion of synthetic material from the fixation of the forehead in one case. Conclucion: In cases with chronic progressive external ophthalmoplegia severe ptosis affecting the daily life and social activity, succesfull surgical intervention can be applied. But, in cases with limited eye motility palpebral apertura should be adjusted in limited measures and patients should be followed regulary in order to prevent postoperative complications. We can suggest that ptotic patients with uncovered visual axis can be followed-up without surgical intervention. | en_US |
