Otoimmun hemolitik anemi ve atopik dermatit birlikteliği olan vaka
Abstract
Otoimmun hemolitik anemi (OİHA), eritrosit antijenlerine karşı sıcak ya da soğuk antikorların oluşması sonucu eritrositlerin yıkılması ile karakterize bir hastalıktır. Literatürdeki vaka sunumlarında, OİHA ile herpetiform pemfigus, dermatomiyozit, dermatitis herpetiformis, astım bronşiale ve anjioödem birlikteliği rapor edilmiştir. Bu yazıda ise otoimmun hemolitik anemi ile atopik dermatit birlikteliği, ilk kez gözlenen bir vaka olarak sunulmaktadır. Dört yaşında kız hasta, halsizlik, cildinde kızarıklık ve döküntü yakınması ile kabul edildi. Fizik muayenede cilt soluk görünümdeydi, yüz ve kollarında papüler lezyonları mevcuttu. Karaciğer 2 cm, dalak 4 cm ele geliyordu. Laboratuvarında Hb: 4.1 g/dL, Hct: % 12 MCV: 105.5 fL, BK: 7,800/mm3, PLT: 191,000/mm3, Retikülosit: % 12 idi. Kemik iliği aspirasyonunda normoblast % 70, myelositer seri normal, megakaryositler (+) idi. LDH: 1529 IU/L, AST: 46 IU/L, IgE: >400 IU/ml, direkt Coombs testi (+) idi. Cilt biyopsisi atopik dermatit ile uyumluydu. Bu bulgularla OİHA tanısı konulan hastaya prednizolon tedavisi başlandı. Tedavinin 7. gününde Hb: 7.0 g/dL'ye yükseldi ve cilt lezyonlarında düzelme gözlendi. Autoimmune hemolytic anemia(AHA) is a disorder resulting from cold and warm antibodies against RBC antigens that destroy these cells prematurely. There are some case reports in the literature which show AHA in association with herpetiform pemphigus, vitiligo, dermatitis herpetiformis, asthma and angioedema. But to the best of our knowledge, this is the first report of AHA in association with atopic dermatitis. 4 year four old girl presented with malaise, hyperemia, and skin/rash. On physical examination skin was pale in apperance and there were papillary lesions on the face and arms. The liver was 2 cm and spleen was 4 cm below the costal margin. Laboratory findings revealed Hb: 4.1 g/dL, Hct: % 12.0 MCV: 105.5 fL, BK: 7,800/mm3, PLT: 191,000/mm3, Reticulocyte: 12 %. Bone marrow aspiration showed that normoblasts were 70 % of cells, myelocytes were normal in apperance and there were enough megakaryocytes. LDH: 1529 IU/L (313-618 IU/L), AST: 46 IU/L (5-40 IU/L), IgE: >400 IU/ml (< 50IU/ml), Direct Coombs test was positive. A diagnosis of AHA was made and prednisolone was started on the 7th day of treatment, Hb increased to 7 g/dL and skin lesions decreased.
