Romatoid artritli hastalarda metotreksat tedavisi: Ön çalışma

Abstract

Bu çalışmanın amacı romatoid artritli hastalarda metotreksat tedavisinin etkinliğinin, etkinliğin ortaya çıkış zamanının, yan etkilerin ve ilaca olan toleransın belirlenmesidir. Çalışmaya ortalama yaşları 52.788+10.109 yıl olan, 18'i(%67.7) kadın 9'u(%33.3) erkek 27 romatoid artritli hasta alındı. Hastalar ortalama 10.333 ±4.416 ay boyunca takip edildiler. Hastaların Stoke indeksi başlangıçta 13.444± 1.333, 12.haftada 8.333±3.354 ve 12. ayda da 3.875±3.314 olarak belirlendi. Stoke indeksi ve eritrosit sedimantasyon hızı açısından istatistiksel olarak anlamlı iyileşme 1 'inci ayın sonundan, CRP açısından 2'inci ayın sonundan, Ritchie skoru ve sabah tutukluğu açısından 3'üncü aydan itibaren ortaya çıkmaya başladı. Toplam 5(%18,4) hastamızda yan etki saptanırken, yan etki nedeniyle 2(%7,4) ve etkisizlik nedeniyle de 2(%7,4) hastamızda tedavi kesildi. Görülen yan etkilerin ve yan etkiden dolayı ilacın kesilme gereğinin düşük oranlarda ortaya çıkması nedeniyle ilaca toleransın iyi sayılabileceği sonucuna varıldı. Bu sonuçlarla metotreksatm romatoid artrit tedavisinde etkin bir ajan olduğu, tedaviye cevabın ortalama 2'inci aydan itibaren ortaya çıktığı, yan etkilerin ilaca toleransın kabul edilebilir düzeyde olduğu düşünüldü.

The aim of this study was to determine the efficiency of methotrexate treatment, the side effects, the time of ap¬pearance of the effects, and the tolerance to drug in pa¬tients with rheumatoid arthritis. Twenty-seven patients with rheumatoid arthritis were admitted to the study with a mean age of 52.788±10.109 years and, 18 (%66.67) of the patients were female and 9 (%33.33) were male. The patients were under observation foraveragely 10.333±4.416 months. Stoke index was determined at onset, at the 12th week and at the 12"'month as 13.444±1.333, 8.333+3.354, and3.875±3.314, respec¬tively. Statistically significant improvement seem to happen, from the point of Stoke index and erytrocyte sedimentation rate, CRP,and Ritchie score and mom-ing stiffness at the end of the first month, beginning from the end of the second month and beginning from the third month, respectively. The treatment was termi¬nated due to the side effects in 2 (7.4%) and due to inef¬ficiency in 2 (7.4%) patients, while the side effects were detected in 5 (18.4%) patients. Since the side effects and the drop out rates due to the side effects were found to be low, we thought that the tolerance to the drug could be acceptable. With these results, it is concluded that methotrexate is an effective agent in the treatment of rheumatoid arthritis, the response to the drug averagely begins from the second month, and the side effects and the tolerance to the drug are at an acceptable range.
