Deneysel sepsis ve nötropenik sepsisli sıçanlarda rG-CSF, İVİG, rG-CSF+İVİG sağaltımının kemik iliği ve dalakta oluşturduğu histopatolojik değişiklikler ve mortalite oranlarına etkisi
Abstract
Bu çalışma E. coli sepsisi geliştirilen 2 aylık dişi sıçanlarda Koloni stimüle eden faktör (rG-CSF), Immun globulin G (İVİG) ve rG-CSF,+İVİG sağaltımının mortalite oranına etkisini, kemik iliği ve dalakta oluşturdukları değişiklikleri saptamak amacıyla yapıldı. Çalışmaya alınan sıçanlar sepsis oluşturulan (A kümesi), sepsis öncesi siklofosfamid verilerek nötropeni oluşturulan (B kümesi) ve yalnız sağaltım uygulanan (C kümesi) olmak üzere üç kümeye ayrıldı. Yedi günlük gözlem süresinde en düşük mortalite oranı A ve B kümelerinde rG-CSF+İVİG verilenlerde saptandı. Ancak bu oran yalnız B kümesinde istatistiksel olarak anlamlı idi. Bu durum rG-CSF'nin nötrofil sayı ve fonksiyonunda iyileşmeye yolaçarak mortaliteyi azalttığını düşündürdü. Kemik ilikleri A ve C kümesinde rG-CSF ve rG-CSF+İVİG sağaltımı verilen sıçanlarda hiperselüler, B kümesi sıçanlarda hiposelüler idi. Ayrıca myeloid/eritroid (M/E) ve immatür myeloid/matür myeloid hücre (İM/MM) oranları kontrollere göre yüksek idi. Bu bulgu, rG-CSF'nin immatür myeloid hücre sayısını artırdığını, İVİG'in tek başına herhangi bir etkisinin olmadığını düşündürdü. Nekroz ve myelofibrozis hiçbirinde gözlenmedi. Tüm kümelerde rG-CSF, rG-CSF+İVİG sağaltımı verilen sıçanların dalaklarının kontrollere göre makroskopik olarak daha büyük olduğu gözlendi. Bu büyümenin artmış ekstramedüller hematopoeze bağlı olabileceği düşünüldü. A, B ve C kümelerinde rG-CSF ve rG-CSF+İVİG sağaltımı verilen sıçanların dalaklarında kordonlarda ve venöz sinüslerde belirgin ekstramedüller hematopoez saptandı. A ve B kümesi sıçanların dalaklarının mikroskopik incelemesinde soluk eozinofilik boyanan selüler debriler gözlendi. Sonuç olarak, rG-CSF ve rG-CSF+İVİG sağaltımı nötropenik sepsiste mortalite oranını azaltmakta ve sağaltım süresini kısaltabilmektedir. Ayrıca kemik iliği M/E ve İM/MM oranlarında belirgin artışa neden olmaktadır. Sepsisli ve nötropenik sepsisli sıçanların dalaklarında artmış ekstramedüller hematopoezis sonucu makroskopik büyüme saptanmaktadır. Bütün bu sonuçlara karşın, klinikte yetişkin insanlarda sepsis sağaltımında rG-CSF'nin tek başına yada İVİG ile kombine kullanımının uygunluğunun belirlenebilmesi için daha çok hayvan deneyleri gereklidir. This study was done to prove the effects of the Colony stimulating factor (rG-CSF), Immune globulin G (IVIG) and rG-CSF+IVIG cure on the rate of mortality and on the bone marrow and spleen in the two-month female rats which were developed sepsis by E. coli. Rats were divided into the three treatment groups; group A, the rats developed sepsis; group B, the rats developed sepsis and neutropenia by administration of cyclophosphamid before inducing sepsis; group C, the rats given therapy only. During the seven day observation period, the least mortality rate was established in rats given rG-CSF+IVIG in either group A and B. But the differences was statistically significant in only group B. It can be speculated that rG-CSF decreased mortality rate by improving neutrophil count and function. The bone marrow of the group A and C rats given therapy with rG-CSF and rG-CSF+IVIG were hypercelular. But the bone marrows of the group B rats were hypocelular. In addition, the rates myeloid/erytroid (M/E) and immature/mature myeloid cell (IM/MM) were higher group A and C than their control groups. These findings indicate that therapy with rG-CSF increased the immature myeloid precursors and therapy with IVIG alone has no effect on these cells. Necrosis and myelofibrosis have not been observed in any treatment group. It was, observed that the spleens of rats having treatment of rG-CSF and rG-CSF+IVIG are bigger macroscopically than those of in their own control group. This changes might be caused by extramedullary hematopoiesis. Specific extramedullary hematopoiesis was proved microscopically in cords and venous sinuses of spleens of the rats having rG-CSF and rG-CSF+IVIG treatment in A, B and C groups. When the spleens of the rats in group A and B were analyzed microscopically, pale eosinophilic stained cellular debris were observed. Based upon these results, therapy with rG-CSF and rG-CSF+IVIG in neutropenic cases may reduce mortality rate and shorten treatment period. In addition, therapy with rG-CSF increased significantly the ratios of M/E and IM/MM and mass of the spleen by extramedullary hematopoiesis. However, further study is needed to clarify the continued efficacy and safety of this new therapy.
