Öğe Futbolcularda Anaerobik Güç Testinin Metabolik Etkilerinin İncelenmesi(2023) Şerare, Serdar; Akyol, Betül; Şenel, Ömer; Çerik, İdris Buğra; Otu, MeryemBu çalışmanın amacı amatör futbolculara uygulanan anaerobik güç testinin metabolik etkilerini tespit etmektir. Çalışmaya yaş ortalamaları; 19,77±1,6 yıl olan, lig döneminde bulunan bölgesel amatör liginden (BAL) ve yerel amatör liginden (YAL) toplam 22 erkek futbolcu gönüllü olarak katılmıştır. Çalışmada anaerobik güç testi olarak wingate anaerobik güç testi (WanT) uygulanmıştır. Test öncesi (TÖ) dinlenik durumda iken ve test sonrası (TS) tüm katılımcıların sol kol dirsek venasından kan örnekleri alınmış ve solunum fonksiyonu testi (SFT) uygulanmıştır. WanT’ın metabolik etkileri, TÖ ve TS elde dilen zorlu vital kapasite (FVC), zorlu ekspirasyon hacmi (FEV1), laktik asit (LA), parsiyel oksijen basıncı (PO2), parsiyel karbondioksit basıncı (PCO2), plazma asit-baz dengesi (pH), bikarbonat (HCO3), oksihemoglobin (O2Hb), karboksihemoglobin (COHb) ve oksijen saturasyonu (SO2) düzeylerinin istatistiksel karşılaştırılmasıyla tespit edilmiştir. İstatistiksel analizleri SPSS 22.0 paket programında gerçekleştirilmiştir. Futbolcuların TÖ ve TS metabolik değişkenlerinin karşılaştırmaları bağımlı örneklem t testi ile, iki ayrı futbol ligi grubunun karşılaştırmaları için bağımsız örneklem t testi gerçekleştirilerek anlamlılık düzeyi (p<0.05) olarak kabul edilmiştir. Sonuç olarak 30 saniye süren WanT’ın amatör futbolcuların FEV1, LA ve PCO2 düzeylerini anlamlı bir şekilde yükselttiği, COHb, HCO3 ve plazma pH seviyesini anlamlı bir şekilde düşürdüğü sonucuna ulaşılmıştır.Öğe Investigation of the effect of intravitreal bevacizumab treatment on left heart function using speckle tracking echocardiography(Sociedade Portuguesa de Cardiologia, 2024) Şahinbaş, Mehmet; Çerik, İdris Buğra; Yalınbaş Yeter, DuyguIntroduction and objectives: Vascular endothelial growth factor (VEGF) inhibitors are widely used in oncology and ophthalmology. Although these agents have been shown to increase the risk of cardiovascular events in systemic use, the effect of local applications is unclear. In our study, we aimed to investigate the effects of anti-VEGF agents on left heart functions after intravitreal injection using speckle tracking echocardiography. Methods: In this prospectively designed study, 44 patients who were going to start intravitreal anti-VEGF treatment were included in the study. Patients were evaluated with speckle tracking echocardiography before the first anti-VEGF administration and at three months of anti-VEGF treatment. Results: Global longitudinal strain (GLS) values at three months were lower in the patients who participated in the study and this was statistically significant (?18.77±2.17, ?18.60±2.01, p=0.001). Also, there was a statistically significant decrease in the mean values of GLS (GLS4CH) obtained from apical four space image, GLS (GLSAPLAX) obtained from apical long axis image and GLS (GLS2CH) obtained from apical 2 space image at month 0 and month 3 (?19.08±2.39, ?18.93±2.26, p=0.004; ?18.81±2.29, ?18.60±2.12, p=0.001; ?18.44±2.31, ?18.27±2.12, p=0.013, respectively). Conclusion: The slight decrease in GLS in our study suggests that the use of intravitreal anti-VEGF agents may have cardiac effects. © 2024 Sociedade Portuguesa de CardiologiaÖğe Is Surgery Really Necessary in Blood Cysts?(Ordu Üniversitesi, 2020) Çerik, İdris Buğra; Bektaş, Osman; Günaydın, Zeki; Dereli, Seçkin; Kaya, AhmetBlood cysts are rare cardiac tumors. Although it is considered a benign pathology, it can also be associated with embolic events. We report 42-year-old woman who admitted to our clinic with the palpitation and dyspnea on exertion. Apical diastolic murmur was present in physical examination. On echocardiography mitral valve posterior leaflet had a typical 'domed' appearance in diastole while anterior leaflet was restricted in motion with a fibrocystic formation on the ventricular side, causing mild to moderate mitral stenosis. We decided medical follow-up with the patient choice and we have been following for two years without any complications.Öğe Koroner Arter Hastalığı Varlığında Periferik Arter Hastalığının Prediktörleri: Plazma Aterojenik İndeks ve Trigliserit Glikoz İndeksi(2023) Koyun, Emin; Şahin, Anıl; Dindaş, Ferhat; Çerik, İdris Buğra; Kanal, Yücel; Karabey, ErcanAmaç: Koroner arter hastalığı olan hastalarda periferik arter hastalığını öngörebilecek basit uygulanabilir parametrelere ihtiyaç va rdır. Aterojenik plazma indeksi ve trigliserit glikoz indeksi ateroskleroz ile ilişkili parametreler olup bu parametrelerin koroner a rter hastalğı olanlarda periferik arter hastalığı öngördürebileceği hipotezi ile çalışmaya başladık. Gereç ve Yöntem: Geriye dönük bir çalışma olup çalışmaya 96 hasta dahil edildi. Hastalar; sadece koroner arter hastalığı olanlar ile ko roner ve periferik arter hastalığı birlikte bulunan hastalar olmak üzere iki gruba ayrıldı. Her iki grup arasındaki değişkenler incelendi. Bulgular: Plazma aterojenik indeks ve trigliserit glikoz indeksi her iki grupta karşılaştırıldı. Yapılan regresyon analizinde AIP ve trigliserit glikoz indeksinın periferik arter hastalığı öngörebilen parametreler olduğu sonucu bulundu. Sonuç: Plazma aterojenik indeks ve trigliserit glikoz indeksi, koroner arter hastalığında periferik arter hastalığını öngörebilen para met relerdir. Koroner arter hastalarında bu indeksler rutin olarak kullanılması önerilebilir. Böylelikle koroner arter hastalarında periferik ar ter hastalığının erken tanı ve tedavisi hızlıca yapılabilecektir.Öğe The Relationship of Inflammation Parameters with Obstruction in Coronary Artery Ectasia(2021) Çerik, İdris Buğra; Dindaş, Ferhat; Ömür, Sefa; Yenerçağ, MustafaBackground: Coronary artery ectasia (CAE) is a coronary artery disease that can progress in a wide clinical spectrum, from asymptomatic cases to stable angina pectoris or acute coronary syndrome. CAE can present with an acute coronary syndrome without an obstructive lesion. Although many studies comparing isolated CAE with normal coronary arteries and coronary artery disease, CAE patients with obstructive type lesions (obstructive CAE) have never been evaluated. In this study, we aimed to compare obstructive CAE patients and isolated CAE patients in terms of various inflammatory parameters. Materials and Methods: A total of 190 patients were enrolled, including 95 obstructive CAE patients and 95 age and sex-matched isolated CAE patients. All participants were evaluated in the study consisted of patients admitted to the emergency department and diagnosed with NSTE-ACS. Systemic inflammatory parameters such as Platelet/lymphocyte ratio (PLR), Neutrophil/lymphocyte ratio (NLR), Monocyte/HDL ratio, CRP and sedimentation of the patients, were compared between two groups. Results: CRP value was found significantly higher in obstructive CAE, compared to isolated CAE [5.5 (3.0- 9.0) mg/L, 4.0 (3.0-6.0) mg/L (P=0.003) respectively]. PLR value was found significantly higher in obstructive CAE compared to isolated CAE [143±76, 120±45 (p=0.015) respectively]. In multivariable logistic regression analysis, only CRP was an independent predictor of obstruction in CAE [OR:1.075 (1.011-1.142), p=0.021]. Conclusions: To the best of our knowledge, this study is the first to evaluate the effects of obstructive lesions on inflammatory parameters in patients with CAE presenting with acute coronary syndrome. We have shown that systemic inflammation is increased in the presence of obstructive coronary artery disease in CAE patients.Öğe Two-year Experience with balloon-Expandable Transcatheter Aortic Valve Replacement in Severe Aortic Stenosis at a Tertiary Center(Sivas Cumhuriyet Üniversitesi, 2024) Şahin, Anıl; Canpolat, Ahmet Caner; Gül, Enes; Çerik, İdris Buğra; Gül, İbrahimObjective Aortic stenosis (AS) is the most common valvular heart disease requiring intervention, particularly in developed countries. Transcatheter aortic valve replacement (TAVR) is indicated for patients with a high surgical risk and a post-procedural survival expectancy of more than 12 months. Over the years, the TAVR method has emerged as a significant treatment option for patients with symptomatic severe AS and has begun to be implemented in our country as well. The objective of this study was to evaluate the short and long-term outcomes of patients undergoing TAVR at our center, as well as to assess our institution's experience with the TAVR procedure. Methods This retrospective, single-center analysis included 16 consecutive patients with symptomatic AS who underwent TAVR between March 2022 and February 2024. All patients included in the study underwent implantation of a balloon-expandable TAVR valve. In the study, the demographic characteristics of patients preoperatively and during post-procedural follow-ups, their clinical status preoperatively and postoperatively, and echocardiographic findings were evaluated and compared. Results The mean age of the entire population was 78.3 ± 8.7 years, and 50% were women. Transfemoral access was used in 93.8% of patients. Implantation success was achieved in all cases. During the TAVI procedure, 12.5% of patients required permanent pacemaker implantation. The mean length of hospital stay for the entire cohort was 4.5±2.3 days. There wasn’t show in-hospital deaths occurred before hospital discharge. During the follow-up, it was observed that 3 patients died from all-cause mortality. The mean follow-up duration of the study was 552 days, with the longest follow-up being 666 days. The significant improvement was noted in all echocardiographic parameters and functional capacity. No cases with moderate or severe aortic regurgitation, necessitating additional procedures. Conclusion Our center results with TAVR over a 2-year span consistent with broader studies. Despite some procedure-related complications, advancements in devices and techniques are expected to reduce these, enhancing outcomes with increased procedural experience. With medicine favoring less invasive approaches, TAVR is poised to become a more prevalent alternative to surgery across diverse patient cohorts.
