Öğe Diagnostic value of autoantibodies against citrullinated peptide antigens in rheumatoid arthritis: Comparison of different commercial kits(Turkish League Against Rheumatism (TLAR), 2011) Şahin, Özlem; Kaptano?lu, Ece; Bakici, Mustafa Zahir; Sezer, Hafize; Elden, Hasan; Hizmetli, SamiObjectives: In this study we tested the diagnostic values of rheumatoid factor (RF), anti-cyclic citrullinated peptide (anti-CCP) and two different commercial anti-mutated citrullinated vimentin (anti-MCV) kits in the differential diagnosis of rheumatoid arthritis (RA) considering that there might also be substantial differences between the performances of the commercial kits. Patients and methods: Thirty-four RA patients, admitted to our rheumatology outpatient clinics between October 2008 and February 2009, and 24 healthy controls were included in this study. Sera of RA patients and healthy controls were analyzed for RF, anti-CCP-2, anti-MCV-548 and anti-MCV-248 autoantibodies with two different commercial kits. Disease activity was determined by disease activity score 28 (DAS-28) in RA patients. Extra-articular involvement was evaluated in RA patients. Results: In the receiver operating characteristic (ROC) curve analysis perfomed to determine the diagnostic sensitivity, anti-CCP-2 had the highest value of area under ROC curve. Sensitivity and specificity was 88% and 90%, 84% and 100%, 80% and 90%, and 84% and 100% for RF, anti-CCP-2, anti-MCV-548, and anti-MCV-248 respectively. DAS-28 had a weak correlation with anti-CCP-2 (r=0.623), anti-MCV-548 (r=0.481), and Anti-MCV-248 (r=0.408). There was no statistically significant difference in RF, anti-CCP-2, anti-MCV-548, and anti-MCV-248 values between patients with or without extra-articular involvement or between patients with low or moderate disease activity according to DAS-28 score. Conclusion: Anti-CCP, RF and anti-MCV autoantibodies are all useful in the differential diagnosis of RA. However, the anti-CCP antibody has a superior diagnostic value compared to the other auto antibodies. The anti-MCV antibodies detected by the anti-MCV-248 kit seem more reliable than the anti-MCV antibodies detected by the anti-MCV-548 kit in differential diagnosis of RA due to their high sensitivity and specificity. These findings suggest that different commercial kits may exhibit different performances. Nevertheless, our results need to be confirmed by future studies which should include more patients. © 2011 Turkish League Against Rheumatism. All rights reserved.Öğe Evaluation of autonomic nervous system dysfunction in fibromyalgia(2011) Şahin, Özlem; Yildiz, Serpil; Yildiz, Nebil; Yaşar, Mustafa Fatih; Kaptano?lu, EceObjective: There have been conflicting results of studies on autonomic nervous system (ANS) dysfunction in fibromyalgia (FM). The aim of this study was to evaluate the sympathetic nervous system dysfunction via sympathetic skin response (SSR) recordings in patients with FM. Materials and Methods: Twenty-seven female patients with FM and 18 healthy volunteer females as controls were included in the study. All participant were administered the Beck Depression Inventory (BDI) and the Beck Anxiety Inventory (BAI) and were questioned about chronic symptoms that are characteristic for FM. Right palmar SSRs were evoked by electrical stimulation of the median nerve at the wrist. Results: The FM patients had higher rates of most of the chronic symptoms of FM. There was no significant difference in SSR latency and amplitude between patients and controls (p>0.05). The patients with FM had no depression and anxiety according to the BDI and BAI mean scores. Conclusion: We did not find ANS dysfunction in the patients with FM. SSR may be an insufficient way of determining the whole ANS dysfunction as it only evaluates the sudomotor component. ANS dysfunction may be responsible for some of the chronic symptoms of FM and accompanying depression and anxiety (not observed in the present study) rather than taking part in the etiopathogenesis. It can be possible to determine the role of ANS in FM etiopathogenesis by studies evaluating the subsystems of ANS (cardiovascular reflex pathway, sympathetic cholinergic activity etc.) in which chronic symptoms of FM, especially anxiety and depression, are also considered. © Turkish Journal of Physical Medicine and Rehabilitation, Published by Galenos Publishing.Öğe Frequency of Neuropathic Pain in Ankylosing Spondylitis Patients(Sivas Cumhuriyet Üniversitesi, 2024) Atik, Seda; Şahin, ÖzlemAnkylosing Spondylitis(AS) is a common chronic inflammatory rheumatological disease. It is known that neuropathic pain, as well as inflammatory pain, increases in rheumatic diseases. In our study, we aimed to investigate the presence of neuropathic pain in AS patients, which we frequently encounter in clinical practice. 52 AS patients were included in the study. HLA-B27 genetic test results, c-reactive protein, and erythrocyte sedimentation rate values of the people included in the study were recorded. LANSS pain scale was applied to all participants to determine neuropathic pain, and Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) was applied to evaluate disease activity. The average age of the individuals participating in the study was 42.57. According to the results of the LANSS pain scale, neuropathic pain was detected in 24 (46.2%) of the individuals. According to BASDAI results, 61.5% of the patients had active disease. A statistically significant relationship was detected between active disease and neuropathic pain (p = 0.001). It was concluded that neuropathic pain may accompany the clinic in a large proportion of patients with AS. We believe that detailed questioning of neuropathic complaints in these patients and treatment if necessary will contribute positively to the quality of life of the patients, and excessive anti-inflammatory treatment can be prevented by reducing the levels of disease activity scales, some of which cannot be evaluated very objectively.Öğe Ultrason tedavisi gören olgularda mikronükleus yöntemi ile ultrasonun genotoksik etkisinin araştırılması(Cumhuriyet Üniversitesi, 2002) Şahin, Özlem; Hizmetli, SamiIV ÖZET ULTRASON TEDAVİSİ GÖREN OLGULARDA MİKRONÜKLEUS YÖNTEMİ İLE ULTRASONUN GENOTOKSİK ETKİSİNİN ARAŞTIRILMASI Tedavi dozlarındaki ultrasonun, mikronükleus (MN) yöntemi kullanılarak insan kromozomlarında hasar yapıcı etkisinin olup olmadığını araştırmak amacı ile yapılan bu çalışmada; bel ağrısı olan ve olmayan 15 kadın olgu çalışmaya alındı. On olgu tedavi uygulanan grupta, 5 olgu ise tedavi uygulanmayan kontrol grubunda yer aldı. Tedavi grubundaki olguların bel bölgesine 1 MHz frekansında 10 dk süre ile 2W/cm2 dozunda 10 seans ultrason tedavisi uygulandı. Olgulardan tedavi öncesinde, 5. seans ve 10. seans sonrasında periferik venöz kan alınarak, MN sayımı için lenfosit hücre kültürüne ekim yapıldı. Kontrol grubundaki olguların ise bel bölgesine 10 dk süre ile akım vermeden (Şam tedavi) 10 seans ultrason probu uygulandı. Tedavi öncesi, 5. seans ve 10. seans sonrasında olguların periferik kanlan alınarak, MN sayımı için lenfosit hücre kültürüne ekim yapıldı. Tedavi ve kontrol grubundaki olguların MN frekansları, tedavi öncesi ile 5. seans sonrası ve tedavi öncesi ile 10. seans sonrası karşılaştırıldı. Tedavi grubu ile kontrol grubunun tedavi öncesi, 5. seans sonrası ve 10. seans sonrası, MN değerleri karşılaştırıldı. Hem tedavi grubunda hem de kontrol grubunda, tedavi öncesi ile 5. seans sonrası ve tedavi öncesi ile 10. seans sonrası MN değerlerinde istatistiksel anlamlı bir fark gözlenmedi. İki grup arasında, tedavi öncesi, 5. seans sonrası ve 10. seans sonrasında MN değerleri karşılaştırıldığında istatistiksel olarak anlamlı bir fark bulunmadı. Sonuç olarak bu çalışmada; tedavi dozlarında uygulanan ultrasonun sitogenetik hasar göstergesi olan MN frekansında artış yapmadığı gözlendi.
