    Aligners from another perspective: Could they be a long-term environmental threat? Problems and potential remedies

    Annual plastic production worldwide has reached unprecedented levels, whereas the amount of plastic being recycled remains significantly low compared with production rates. This trend is similarly reflected in the use of clear aligners, which are becoming increasingly popular. The purpose of this review, which was one of the topics discussed at the AAO 2023 Winter Conference, is to raise awareness among patients and orthodontists regarding the environmental impact of aligners, assess whether aligners pose a threat to our planet, and propose potential solutions to the existing challenges.     Assessment of mandibular trabecular structure and cervical vertebral maturation using fractal analysis

    BACKGROUND: Changes in bone trabeculae occur during skeletal maturation. Fractal analysis is a technique for assessing changes in the internal structure of the bone. OBJECTIVE: The aim of this study was to investigate correlation of bone trabeculation measured on panoramic and lateral cephalometric radiographs with cervical vertebral maturation stages (CVMS). METHODS: CVMS was evaluated on lateral cephalometric radiographs of 120 individuals aged 6 to 18 years. The subjects were divided into 6 groups (n = 20 each) according to CVMS. On panoramic and lateral cephalometric radiographs, fractal dimension (FD) analyses were performed in mandibular corpus, gonial and condylar regions and cervical vertebral bodies (C2, C3, C4). RESULTS: FD measurements from the mandibular corpus region bilaterally and C2-C4 vertebral bodies showed significant positive correlations with CVMS (r = 0.418, r = 0.412, r = 0.324, r = 0.304, r = 0.263, respectively). Significant differences were found in the FD values of the right/left mandibular corpus and C2 and C3 vertebrae compared to CVMS (P < 0.001, P < 0.001, P = 0.005, P = 0.019, respectively). CONCLUSION: Significant positive moderate correlations between the FD values of the right and left mandibular corpus and CVMS suggest that fractal analysis may be useful in determining skeletal maturity stage. The significant FD values obtained from the right/left mandibular corpus, and C2 and C3 vertebra indicate that the complexity of the internal trabecular structure increases with maturation.

    Dental Anxiety and Fear Levels, Patient Satisfaction, and Quality of Life in Patients Undergoing Orthodontic Treatment: Is There a Relationship?

    Objective: this study aimed to evaluate the relationship between dental anxiety and fear levels, patient satisfaction, and oral health-related quality of life (OHROoL) in patients undergoing orthodontic treatment. Methods: The study was conducted in the Department of Orthodontics, Sivas Cumhuriyet University Faculty of Dentistry. This cross-sectional study included 252 patients, aged 11-14 years undergoing orthodontic treatment. The data collection tools were the Index of Dental Anxiety and Fear (IDAF-4C(+)), the Child Perceptions Questionnaire (CPQ 11-14), the Patient Satisfaction Questionnaire, and the Clinical Examination Data Form. The CPQ 11-14 was used to measure OHROoL. Descriptive statistics, the independent samples t-test, analysis of variance (ANOVA), Cronbach's alpha, and Pearson coefficient were used for statistical analysis at a significance level of 0.05. Results: The CPQ 11-14 parameters were sufficiently reliable, and the patients mostly had problems with oral symptoms. A significant difference was observed between the type of treatment, the initiation of treatment, emotional well-being, and social well-being (P < .05).The relationship between treatment satisfaction and all parameters was significant (P < .05). There was a significant relationship between IDAF-4C(+) and CPQ 11-14, while a moderate correlation was found between dental anxiety and emotional well-being. Conclusion: According to the results of this study, the type of treatment, the initiation of treatment, and dental anxiety impact the quality of life. It was found that treatment satisfaction and a positive patient-dentist relationship positively affect the quality of life and dental anxiety.

    Evaluation of knowledge, approaches, and self-efficacy perceptions of Turkish dental students about preventive and interceptive orthodontic applications in 2018-2019

    BACKGROUND AND AIM: Preventive orthodontics aids in the formation of normal occlusion. There have been numerous studies on this topic published in the literature. The purpose of this study was to assess final-year dental students' knowledge, attitudes, and self-efficacy perceptions of preventive and interceptive orthodontic applications (PIOA). METHODS: Data were collected from 410 dental students from eight different faculties in this cross-sectional study using a predesigned and validated self-administered, structured questionnaire. SPSS software was used to analyze the data, which included descriptive statistics, the independent samples t-test, analysis of variance (ANOVA), and the KuderRitchardson Formula 20 (KR-20) reliability coefficient. The statistical significance level was set at P <= 0.05. RESULTS: The vast majority of students (71.0%) did not believe that they were qualified to perform PIOA after graduation. With a rate of 80.5%, preventive treatment was chosen as the most important treatment type. The most correctly answered question, with a score of 92.4%, was about space maintainers. The total score was calculated to be 9.13 +/- 2.73. There was a significant difference in total scores between men and women (P = 0.0340). There was a significant difference in total scores between those who thought and those who did not think that enough time had been allocated to theoretical education (P = 0.0001). The total score differed significantly from the responses to the question Do you believe you have sufficient theoretical and practical knowledge about PIOA? (P = 0.0001). CONCLUSION: The women' knowledge level was higher than that of the men, and the students valued preventive measures. Experts should consider these findings when developing the core curriculum.

    Orthodontic Approach to Patients with Autism: A Review

    Autism is a complex neurobehavioral disorder that causes problems in thinking, feeling, language, and the ability to communicate with other people. Over the past two decades, there has been a great deal of interest in autism disorder. This problem is recognized worldwide, and various measures are taken both nationally and internationally to improve the lives of affected individuals and families. The oral health care of such patients can be complicated since they cannot express their problems and can show unpredictable behaviors during treatment. The aim of the literature review was to collect information about the general oral health status of patients with autism, the difficulties that may be encountered during treatment, and the precautions that can be taken and the treatment approaches. This article reviews the dental literature from 1943 to the present.

    Shear bond strength of orthodontic brackets to Zirconium oxide infrastructure treated with Er:YAG, Nd:YAG, and KTP lasers: An experimental study

    Objective: The aim of the present study was to investigate the shear bond strength of orthodontic brackets to zirconium oxide infrastructures treated with erbium-doped yttrium aluminum garnet (Er:YAG), neodymium-doped yttrium aluminum garnet (Nd:YAG) and potassium titanyl phosphate (KTP) laser modalities in in vitro settings. Materials and Methods: A total of 40 zirconium oxide infrastructures were prepared with CAD/CAM technology in accordance with ISO 11405 standard. The specimens were divided into 4 groups as following: Er:YAG, Nd:YAG, KTP, and control groups (n=10). Prior to the application of cementation of orthodontic brackets, the surfaces of the zirconium oxide infrastructures were irradiated using selected laser modailites. Shear bond strength tests were performed on each specimen by using a universal testing machine. Results: The shear bond strength value of Er:YAG laser group was significantly higher than those of all other groups (p<0.05); although the bonding strength of Nd:YAG laser was higher than that of the KTP laser, this difference was not reached statistical significance (p>0.05). The bonding strength values of Nd:YAG laser group were significantly higher than that of the control group (p<0.05); and the the bonding strength values of KTP laser group were significantly higher than that of control group (p<0.05). Conclusions: The bond strength between the orthodontic brackets and zirconium oxide infrastructures was improved upon using all the laser modalities in the present study, among which, application of the Er:YAG laser was the most successful. © 2020, Cumhuriyet University Faculty of Dentistry.

    The effects of orthodontic treatment type, depression, and treatment need on perceived pain intensity

    Objectives: The objectives of this study were to evaluate how orthodontic treatment type, treatment need, and depression affect perceived pain intensity (PPI). Material and Methods: This prospective study included 172 patients (80 girls and 92 boys, mean age 14.32 +/- 1.57 years) treated at the Sivas Cumhuriyet University Faculty of Dentistry, Department of Orthodontics. The patients were divided into three groups: the first group was treated with fixed orthodontic appliances, the second group was treated with twin blocks, and the third group was treated with rapid maxillary expansion and reverse headgear (RME/RH). Data were collected and classified with the Index of Orthodontic Treatment Need-Dental Health Component (DHC), Children's Depression Scale, and Visual Analog Scale. PPI was recorded in the 1st week (T0), 2nd week (T1), the 1st month (T2), 2nd month (T3), 3rd month (T4), and the 6th month (T5). Results: PPI was higher in boys at T1 (P = 0.005) compared with girls. There was a significant difference of PPI between the treatment groups at T2 (P = 0.036), T3 (P = 0.012), T4 (P = 0.000), and T5 (P = 0.006). A statistically significant positive correlation was found between DHC and PPI at T3 (r = 0.182; P = 0.000), T4 (r = 0.161; P = 0.03), and T5 (r = 0.189; P = 0.000) time periods. There was no significant correlation between depression and PPI. Conclusion: Girls were more resistant to pain than boys. The type of treatment and the need for treatment both had an effect on PPI. Further studies are needed to investigate the effects of psychological states on PPI during orthodontic treatment.
