Öğe A rare entity: Penile epidermoid cyst in a boy(Sivas Cumhuriyet University, 2021) Cankorkmaz, Levent; Eğilmez, ReyhanEpidermoid cysts (EC), is one of the common benign tumours presenting anywhere in the body. However penile EC is rare, even rarer in pediatric age group. There are usually congenital. EC result from the proliferation of epidermal cells within a circumscribed space of the dermis.A five-year-old boy presented with an asymptomatic, elastic swelling on the ventral surface of the penis near the corona for three years. The swelling was flesh-colored, subcutaneous, and freely movable within the dermis. He had no history of inflammation or any urinary symptoms. Rest of the genitals were normal. Excision of the cyst was performed under general anesthesia. The histopathology reported the swelling as an EC. No recurrence has been noted in the three year since excision.Penile EC can occur after minor surgical procedure like circumcision. The condition needs to be treated with total surgical excision in case it occurs.Öğe Bir Tıp Fakültesinde Öğrencilerin Kanıta Dayalı Tıp Yaklaşımları(Tıp Eğitimini Geliştirme Derneği, 2020) Ağadayı, Ezgi; Eğilmez, Reyhan; Altun, AhmetAmaç: Cumhuriyet Üniversitesi Tıp Fakültesi (CÜTF) öğrencilerinin Kanıta Dayalı Tıp (KDT) konusundaki bilgi ve tutumlarını tanımlamak ve dördüncü dönem öğrencilerine verilen 1 haftalık KDT eğitimi hakkında görüşlerini belirlemektir. Gereç ve Yöntem: CÜTF üçüncü, dördüncü ve beşinci dönem öğrencilerinin dâhil olduğu tanımlayıcı bir çalışmadır. Dördüncü dönem verilen 1 haftalık KDT eğitimi, teorik dersler ve pratik yapılan bir stajdır. Araştırmada kullanılan veri toplama aracı öğrencilerin tanımlayıcı özelliklerini, KDT hakkında bilgi ve tutumlarını ve staj hakkında görüşlerini sorgulamaktadır. Bulgular: 165 öğrenci katılmak için gönüllü oldu. Araştırma sorusunu araştırmak için belirtilen en yaygın kaynak; dönem üç öğrencileri için genel internet siteleri, dönem dört ve beş öğrencileri için elektronik KDT veri tabanlarıdır. Dönem üç öğrencileri veri tarama için kolay ulaşıma önem verirken, dönem dört ve beş öğrencileri için güvenilirlik önemliydi. Öğrencilerin araştırma yapmak için elektronik veri tabanlarını kullanım sıklığı (p<0,001), anahtar kelimeyi “iyi” olarak bilenlerin oranı (p=0,021), Medline kullanım sıklığı (p=0,003) ve kanıt piramidinden aldıkları puanların ortalaması (p<0,001) dönemler arasında anlamlı farklılık göstermektedir. Öğrencilerin İngilizce seviyeleri ile elektronik KDT veri tabanı kullanımı arasında anlamlı fark vardır (p=0,034). Ortanın üzeri ve üst seviye İngilizcesi olan öğrencilerin %72,7 (8)’si araştırma yapmak için elektronik KDT veri tabanlarını kullanmaktadır. Öğrencilerin makale okurken en zorlandıkları kısımlar; İngilizce yetersizliği, istatistik ve makalede kullanılan akademik dil idi. Dönem 4 ve 5 öğrencilerine KDT stajı hakkında görüşlerini sorduğumuzda; % 77,2 (88)’si stajın yararlı olduğunu, % 7,0 (8) öğrenci kursun faydalı olmadığını, % 15,8 (18)’i ise kararsız olduğunu belirtti. Sonuç: Öğrencilerin İngilizce düzeylerini iyileştirmek KDT veri tabanı kullanım sıklıklarını artırabilir. KDT bilgi ve tutumunun dönem üç öğrencilerinde diğer sınıflara göre oldukça düşük olduğu, stajını yeni alan dönem dört öğrencilerinin genel KDT bilgi ve tutumunun diğer sınıflara göre daha iyi olduğu görülmüştür. Bu bulgular bize stajın faydalı olduğunu göstermektedir.Öğe Kaposi Sarcoma: Single Center 10 Years Of Experience(Manisa Celal Bayar Üniversitesi, 2025) Kaya, Şeyhmus; Aydın, Sedanur; Yüceer, Ramazan Oğuz; Koç, Tülay; Eğilmez, ReyhanAim: Kaposi sarcoma is a rare angioproliferative disease associated with human herpesvirus-8. There are limited studies in our country regarding its epidemiology and clinical features. This study aims to explore cases diagnosed with Kaposi sarcoma at Sivas Cumhuriyet University Medical Faculty Research Hospital. Materials and Methods: The records of patients (n:18) diagnosed with Kaposi sarcoma between January 1, 2013, and June 30, 2023, in the Department of Medical Pathology were retrospectively reviewed. When recurrences were considered, the total number of cases increased to 27. The clinical, histopathological, and demographic characteristics of the patients were examined. Results:The mean age of the patients was 69.3±13.3 years (range:32–88). Thirteen patients (72.2%) were male, and five (27.7%) were female. None of the patients had a history of HIV. Of the lesions, 66.6% (n=12) were localized in the lower extremities, 22.2% (n=4) in the upper extremities, and 11.1% (n=2) in non-extremity regions. Histopathological examination revealed that 72.7% (n=13) of the patients were in the tumor (nodule) stage, 22.2% (n=4) in the patch stage, and 5.6% (n=1) in the plaque stage. All patients were treated surgically. Among the recurrence cases, a total of nine cases involved five patients. Additional diseases observed in the patients included heart disease, hypertension, and diabetes. Conclusion:To better evaluate the demographic data and clinical features of Kaposi sarcoma in our country, multicenter studies are needed, as well as increased awareness of this disease among relevant physicians.Öğe Ki-67 Proliferation Index Positivity Limit (Cut-off) Value in Meningiomas(Bolu Abant İzzet Baysal Üniversitesi, 2023) Yeldir, Neşe; Doğan, Ezgi; Özer, Hatice; Söylemez, Burçak; Çelikgün, Serkan; Eğilmez, ReyhanObjective: Meningiomas are usually slow-growing grade 1 tumors according to World Health Organization (WHO) classification. Histological and molecular criterias were defined for the determination of meningiomas of high grade and more aggressive. The proliferation activity is also considered among histopathological criterias. A commonly used parameter for determining proliferation activity is the Ki-67 index. This study was formed retrospectively to identife the positivity limit (cut-off) value of the Ki-67 proliferation index in cases diagnosed with meningioma. Materials and Methods: The Ki-67 indices of a total 190 patients diagnosed with meningioma was compared with the histological degrees of the tumors. The ROC analysis method was used in the statistical analysis for Ki-67 values. Results: The mean Ki-67 proliferation index was found to be 6.3% for grade 1, 11.2% for grade 2 and 26% for grade 3 in the cases diagnosed with meningioma in our center. The Ki-67 proliferation index, which can be used in addition to histopathological findings in the distinction of grade 1 and grade 2 meningiomas in our laboratory, was determined as a positivity limit value of 6.5 (p>0.05). Conclusion: The Ki-67 proliferation index in meningioma is an important parameter that can be used to determine the histological grade of atypical meningioma in particular. Although the index seems to be easy to use, it is a value that can create difficulties in borderline cases and also in interobserver and intraobserver evaluations. Therefore, each pathology laboratory should determine the Ki-67 value that they can accept as a limit for meningioma grading.Öğe Patient With A Diagnosis Of Ochronosis After Lumbar Dyscectomy: Case Report(Sivas Cumhuriyet University, 2020) Yeldir, Neşe; Küçükyıldız, Halil Can; Başpınar, Nisa; Özüm, Ünal; Eğilmez, ReyhanOchronosis is the black discoloration of connective tissues seen with alkaptonuria, a metabolic disorder. Alkaptonuria is a rare autosomal recessive metabolic disorder caused by the lack of homogentisic acid oxidase enzyme. Alkaptonuria causes degenerative changes in cartilage, intervertebral disc and other tissues. The patients operated due to lumbar disc herniation in alkatonuria are very few. In this article a case of ochronosis in which the patient was determined after lumbar discectomy is presented.Öğe Prognosis in IDH-Mutant and IDH-wild type glioblastoma(Sivas Cumhuriyet University, 2021) Kılıç, Eren; Yeldir, Neşe; Özüm, Ünal; Yücel, Birsen; Yılmaz, Mukaddes; Özer, Hatice; Eğilmez, ReyhanGlioblastoma (GBM) is the most common brain tumor in adults with a poor prognosis and predominantly astrocytic differentiation. GBM is classified according to the isocitrate dehydrogenase (IDH) mutation according to the World Health Organization (WHO) Brain Tumors 2016 classification. IDH-1 gene mutations are evaluated immunohistochemically (IHC) and molecular methods in current pathology laboratory practice.The aim of this study was to determine the overall survival in GBM cases according to IDH-1 mutation status and Ki-67 proliferation index.Total of 53 patients diagnosed as GBM between 2007-2020 in Sivas Cumhuriyet University Faculty of Medicine, Department of Pathology were evaluated. Disease-free median survival of 7 (13%) GBM patients with IDH mutation was 2 months (2-year survival 14%), disease-free median survival 9 months (2-year survival 24 %) in GBM IDH-wild type cases. In cases with a higher Ki-67 proliferation index, the median disease-free survival were shorter.In conclusion statistically significant results could not be obtained between disease-free survival in GBM IDH-mutant type and GBM IDH-wild type. However, the mean Ki-67 proliferation index of GBM IDH-wild type cases was found to be much higher than GBM IDH-mutant cases. High Ki-67 proliferation index was found to be associated with a significantly shorter survival.
