Öğe A Cross-Sectional Survey Study on the Use of Communication Methods in the Dentist-Geriatric Patient Relationship(Cumhuriyet University Faculty of Dentistry, 2022) Güler, Kübra; Görgün, Emine PirimObjectives: The population has been aging more rapidly than in the past, and it has been reported that the number of people over the age of 60 exceeds the number of children under the age of 5. This demographic change has forced countries to plan their health systems with the aging population in mind. In this study, we aimed to see and evaluate the attitudes of dentists and trainee dentists towards communication with their elderly patients during their examination and treatment. Materials and methods: We applied a 25-item questionnaire on dentist-geriatric patient communication to 241 participants, including the 4th and 5th-year students, residents, and faculty members. Sixteen items, which of the first 7 are "basic", include questions that examine the methods recommended by the American Medical Association in physician-patient communication and whether they find these methods effective. The data we obtained from the survey results were analyzed with appropriate statistical methods. Results and Discussion: Dentists routinely used an average of 4.6 of the 16 methods and 2.5 of the seven basic methods. While the most frequently used methods were “speaking slowly” and "using a simple language", the least was "using videos". Health literacy awareness and outcome expectancy were associated with the number of methods used. Conclusions: The number of routine use of the methods is quite low among dentists and trainee dentists. It has been observed that communication methods that would be effective in relationships with geriatric patients with low health literacy skills are not routinely used. It has been concluded that the communication methods that can be used effectively for communication with geriatric patients should be embedded into the dental curriculum. © This work is licensed under Creative Commons Attribution 4.0 International LicenseÖğe Agresif periodontitis hastalarında interlökin-6 ve interlökin-10 gen polimorfizmlerinin saptanması ve başlangıç periodontal tedavinin klinik, dişeti oluğu sıvısı ve serum parametreleri üzerine etkileri(Cumhuriyet Üniversitesi, 2014) Görgün, Emine Pirim; Toker, HülyaSitokinlerde gözlenen genetik varyasyonlar periodontitise duyarlılığı etkilemektedir ve kompleks etyolojiye sahip bir hastalık olan periodontitiste gen polimorfizmleri hastalığın gelişimine katkıda bulunabilir. Literatürde hangi genlerin ya da varyasyonlarının ne derecede hastalığa katkıda bulunduğu konusundaki bilgiler çelişkilidir. Bu çalışmada, agresif periodontitis (AgP) hastalarında interlökin-6 (IL-6) (-174) ve interlökin-10 (IL-10) (-597) gen polimorfizmlerinin periodontal ve sistemik olarak sağlıklı kontrol (K) grubu ile karşılaştırılması, polimorfizm saptanan bireylerde başlangıç periodontal tedavinin dişeti oluğu sıvısı (DOS) ve serumdaki IL-6 ve IL-10 düzeylerine etkisini değerlendirmek amaçlanmıştır. 53 AgP hastası ve 50 periodontal açıdan sağlıklı birey kontrol grubu olarak çalışmaya dahil edildi. Başlangıç ve 6.haftada klinik periodontal ölçümler (plak ve gingival indeks, sondalama cep derinliği, klinik ataşman seviyesi) ile DOS ve kan örnekleri alındı. AgP grubundaki hastalara başlangıç periodontal tedavi uygulandı. IL-6 ve IL-10 gen polimorfizmi PCR-RFLP(polimeraz zincir reaksiyonu - restriksiyon parça uzunluk polimorfizmi) ve sitokin düzeyleri ELİZA yöntemi ile belirlendi. AgP grubunda tedavi sonrası klinik parametrelerde anlamlı iyileşme gözlendi(p<0.05). AgP grubunda IL-6 GG genotipi ve G alel sıklığı K grubuna göre daha yüksek oranda saptandı. AgP grubunda DOS IL-6 sitokin düzeyinde başlangıca göre 6.haftada istatistiksel olarak anlamlı azalma saptanırken, IL-6 GG genotipli bireylerde başlangıç DOS IL-10 düzeyi GC+CC genotipli bireylere oranla daha yüksekti(p<0.05). Genotip dağılımların DOS ve serum IL-6 ve IL-10 düzeylerine etkisi ile ilgili diğer karşılaştırmalarda anlamlı değişiklikler saptanmadı(p>0.05). Bu çalışmanın sınırları dahilinde, IL-6 (-174) ve IL-10 (-597) gen polimorfizmleri agresif periodontitis ile ilişkili bulunmuş fakat genotip dağılımları (IL-6 GG, IL-10 AA genotipi) periodontal tedavi cevabı üzerine klinik açıdan değişikliğe neden olmazken, IL-6 GG genotipi DOS IL-10 düzeyinde artışa neden olmuştur.Öğe COMPARISON OF ORAL HYGIENE ATTITUDES, AWARENESS AND PERIODONTAL PARAMETERS OF UNDERGRADUATE DENTAL STUDENTS(2019) Alpan, Aysan Lektemur; Görgün, Emine PirimObjectives: The aim of this study was to compare the differences in selfreported oral health attitudes and clinical measurements between undergraduatedental students who enrolled in or not enrolled in periodontology course inTurkey.Materials and Methods: A total of 701 students were included in the study.Students asked 26 questions including age, gender, smoking, oral hygiene habitsand self-assessment measures for periodontal status. Probing depth (PD) clinicalattachment level (CAL), presence of bleeding on probing (BOP), plaque index(PI) and gingival index (GI) measurements performed. The Chi-square test wasused for categorical data and one way Anova post hoc Tukey test for ordinal leveldata.Results: There were no differences between grades in term of PD and CAL(p>0.05). Statistically significant difference was in BOP value between 1st and 5th grade (p<0.05). PI values of 1st grade were statistically higher than 3rd, 4th and 5th grades (p<0.05). GI of 1st grade was statistically higher than 4th and 5th grades(p<0.05). GI of 2nd grade was statistically higher than 3rd, 4th and 5th grades(p<0.05). Tooth brushing was not different between grades (p>0.05). Interdentalcare ratios in grades significantly differ from each other (p<0.05). According tostudents, they did not have any kind of periodontal disease with high percentagesabove 88%. 4th and 5th grades had periodontal treatment comparing the othergrades (p<0.05).Conclusions: Starting to take periodontology course in dental faculties from thefirst year and constituting a periodontal disease prevention program will bebeneficial to students in Turkey.Öğe GCF Levels of Osteoclastogenesis-Related Cytokines in Periodontitis in Relation to Smoking During Non-Surgical Periodontal Therapy(Ordu University, 2023) Erenler, Nilüfer; Görgün, Emine PirimObjective: Interleukins (IL) -1β, -34, receptor activator of nuclear factor-kB ligand (RANKL), and osteoprotegerin (OPG) play a crucial role in osteoclastogenesis and bone resorption through modulating inflammatory processes and osteoclastogenesis. Smoking is the major risk factor in the initiation and progression of the periodontitis, and adversely affects the outcomes of non-surgical periodontal therapy. To date, there is no study investigating both gingival crevicular fluid (GCF) IL-1β, IL-34, RANKL, and OPG levels before and after non-surgical periodontal therapy in smoking and non-smoking patients with periodontitis stage 3, grade B and C. The aim of current research was to examine the GCF levels of some osteoclastogenesis-related cytokines in periodontitis in relation to smoking before and after periodontal therapy. Methods: At baseline, full-mouth periodontal status together with GCF samples were collected from 116 individuals, including 60 periodontitis patients (30 smokers and 30 nonsmokers) and 56 periodontally healthy controls (28 smokers and 28 nonsmokers). Non-surgical periodontal therapy, consisting of instruction for daily plaque control and scaling and root planing (SRP), was performed. GCF sampling and full-mouth periodontal measurements were repeated 6 weeks after completion of SRP. The GCF levels of biomarkers were measured by enzyme-linked immunosorbent assay. Results: The periodontitis groups exhibited significant improvement in clinical parameters. At baseline, the GCF IL-1β levels in periodontitis groups were significantly higher than periodontally healthy controls (p<0.05) and it was significantly decreased in periodontitis groups after non-surgical periodontal therapy. At baseline, the GCF IL-34 levels in periodontitis groups were significantly higher than periodontal healthy controls(p<0.05) and the GCF IL-34 level was significantly decreased in non-smoking periodontitis patients. At baseline and after periodontal therapy, the GCF RANKL levels were similar in all groups. The GCF OPG level was significantly lowest in non-smoking periodontitis patients at baseline and the GCF OPG level was significantly increased in smoking and non-smoking periodontitis patients after non-surgical periodontal therapy. Conclusion: In the periodontal inflammation process, GCF IL-34 level followed a similar pathway to GCF IL-1β, suggesting that IL-34 may be a marker in the pathogenesis of periodontal disease. The significant decrease in GCF IL-34 and a significant increase in GCF OPG level in the non-smoker periodontitis group after periodontal therapy suggest the negative effect of smoking on the response to periodontal therapy. More comprehensive studies are needed by increasing the number of samples included in the study groups in order to better understand the pathogenesis of periodontitis.Öğe Preterm birth and periodontitis(Nova Science Publishers, Inc., 2023) Görgün, Emine PirimBacterial plaque is primary etiologic agent in periodontal diseases. It is well known that host related factors such as genetic, systemic diseases affect the periodontium, especially the gingiva and alveolar bone. Today, the effect of periodontal disease on systemic health has been proven by many studies. Many prominent changes occur in the innate and adaptive immune system with pregnancy. Preterm birth, which is the most important known pregnancy complication today, is a serious problem for developing countries. The most important step in managing preterm birth is to know the risk factors and treatment strategies. One of the factors that may affect the healthy maintenance of the fetal-maternal unit is maternal periodontitis. The new studies aim to explore the mechanisms by which periodontitis affects the etiology of preterm birth. In this chapter, the role of periodontal disease in preterm birth, the relationship between maternal periodontal disease and preterm birth, and current information on the management of the process have been criticized. © 2023 Nova Science Publishers, Inc.
