Öğe A Case of Oropharyngeal Tularemia Mimicking Lymphoma During Pregnancy(Ankara Microbiology Soc, 2022) Elaldi, Nazif; Oz, Murtaza; Kurt, Begum; Turgut, Ismail; Koc, Tulay; Turan, Meral; Buyuktuna, Seyit AliTularemia is a zoonotic bacterial infectious disease caused by a gram-negative coccobacillus namely Francisella tularensis. In humans, disease leads to several different clinical forms (ulceroglandular, glandular, oculoglandular, respiratory, typhoidal and oropharyngeal). Since the main mode of transmission of the disease to humans in Turkiye is by drinking water contaminated with F.tularensis, the oropharyngeal form is the most common clinical manifestation. Since tularemia cases with pregnancy are rare, the literature about maternal and fetal complications of tularemia is sparse. In this report, a case of oropharyngeal tularemia mimicking lymphoma during pregnancy was presented. A 33-year-old 11-week pregnant patient living in a village in Sivas province admitted to the infectious diseases and clinical microbiology outpatient clinic with the complaint of swelling in the neck region that continued for six days. The patient, who was engaged in animal husbandry stated that she consumed raw milk and admitted to the otorhinolaryngology outpatient clinic of a hospital 10 days ago with the complaints of fever, chills, and sore throat. She stated that her complaints did not regress with the amoxicillin-clavulanate treatment recommended by her doctor and she noticed the swelling in her neck on the 4th day of the treatment. Upon further questioning, it was understood that the patient had a history of consumption of unchlorinated spring water. Her vital signs were normal and physical examination revealed non-fluctuant lymph nodes with the largest of 5 x 2 cm in the right posterior cervical region, and 3 x 2 cm in the left. Laboratory tests revealed a blood leukocyte count of 13.32 x 103/mm3 (75% granulocytes), a blood hemoglobin of 11.4 g/dL, an erythrocyte sedimentation rate of 45 mm/hour, and C-reactive protein of 90 mg/L. A non-contrast MRI examination revealed wall thickening of the nasopharynx and enlarged lymph nodes which were suspicious for lymphoma with significant diffusion restriction on diffusion -weighted images. As the past medical history and clinical findings were suggestive for tularemia, the microagglutination test (MAT) was studied, but it was reported as negative with a titer at 1/80. Since the patient's complaints continued and tularemia cases were encountered in our region in the past years, the repeated MAT after two weeks was reported as positive with a titer at 1/320. An oropharyngeal form of tularemia was diagnosed and oral ciprofloxacin (2 x 750 mg) was given for three weeks by starting at the 14th gestational week. Lymphoma was excluded by histopathological examination of the fine needle aspiration biopsy performed on the patient's cervical lymph nodes, but the biopsy sample was compatible with granulomatous diseases. Histopathological findings of diagnostic biopsies of the larynx and nasopharynx were reactive. A healthy male baby, 2425 grams, 47 cm, was delivered by cesarean section from the patient who presented with labor contractions at the 37th week of pregnancy. There was no sign of congenital infection in the newborn. The patient and the baby were followed up to the end of one year and no abnormality was found. The evaluation of 17 cases reported in the literature including this case, suggest that tularemia may progress to involve serious obstetric complications during pregnancy, such as abortion, premature birth and intrauterine fetal death when appropriate and effective antibiotic treatment is not given.Öğe ANALYSIS OF CANCERS IN A UNIVERSITY HOSPITAL EMPLOYEES BETWEEN 2006 AND 2022(Dokuz Eylul Univ Inst Health Sciences, 2023) Celikgun, Serkan; Koc, Tulay; Ucku, Serife ReyhanPurpose: Healthcare workers are exposed to a variety of hazardous chemicals and agents that may have long-term effects on their health. One of the health problems caused by these chemicals and agents is cancer. The aim of this descriptive study is to evaluate the cancers seen in those working in a university hospital between 01.01.2006 and 30.06.2022.Material and Methods: All healthcare professionals diagnosed with cancer between 01.01.2006-30.06.2022 were included in this descriptive study. The age, sex, occupation, working unit, and cancer type of the patients were evaluated. Data were analyzed with the statistical program SPSS-22 (SPSS Inc., Chicago, IL, USA). Data were presented using descriptive statistics (means and standard deviations for continuous variables; numbers and percentages for categorical variables).Results: Between 01.01.2006 and 30.06.2022, 186 people working at Sivas Cumhuriyet University were diagnosed with cancer. Forty-seven point eight percent (n = 89) of them are male; the mean age of those diagnosed with cancer is 46.0 +/- 8.9 (min 22, max 62). The most common cancers are thyroid (21.5%), breast (16.7%), head and neck (9.7%), hematopoietic system (9.1%), and urinary system (8.1%) cancers. When evaluated according to occupational groups, 22.6% of them are office workers, 19.9% are academicians, 15.6% are workers, 14.0% are nurses, 10.2% are health technicians, and 8.2% are research assistants.Conclusion: In this institution, which has 2721 personnel, approximately 12 employees are diagnosed with cancer each year. Detailed records of patients diagnosed with cancer, especially healthcare professionals, should be kept. More studies are needed in detail on behaviors related to occupational exposures and cancer risk.Öğe Biocompatibility of different resin composites after polymerization with two light curing units: an immunohistochemical study(Istanbul Univ Press, Istanbul Univ Rectorate, 2023) Ipek, Irem; Unal, Murat; Koc, TulayPurpose The aim of this study is to compare the biocompatibility of two different resin composites after polymerization under two different light sources in three different time periods. Materials and Methods 72 polyethylene tubes polymerized with 2 different resin composites and 2 different light sources (Elipar S10 and Valo ) [Group 1: Kalore Elipar S10 (KE), Group 2: Kalore Valo (KV), Group 3: Essentia Elipar S10 (EE), Group 4: Essentia Valo (EV)] were implanted in the dorsal connective tissue of 18 rats. 24 empty polyethylene tubes [Group 5: (Control group)] were implanted in the dorsal connective tissue of 6 rats. Then, the rats were sacrificed after 7th, 15th and 30th days in each time intervals (n=8). Biopsy samples were stained with H&E and examined for inflammation, necrosis, macrophage infiltrate, giant cell and fibrous capsule criteria. Immunohistochemical staining was performed to evaluate proinflammatory cytokines (IL-1 beta, IL-6 and IL-8). Results When the composite groups and the control groups were compared; there was difference statistically significant for the criteria of inflammation at 7th and 15th days, there was no statistical difference between the time points in terms of fibrous capsule and necrosis. When the composite groups and control groups were evaluated in terms of proinflammatory cytokines; statistically significant differences were found at 7th, 15th and 30th days. Conclusion All CRs used in this study showed acceptable biocompatibility in the subcutaneous tissues of rats after polymerization with different light sources.Öğe Evaluation of the prognostic role of androgen receptor positivity in breast cancer.(ALLIED ACAD, 2017) Erdis, Eda; Koc, Tulay; Yucel, Birsen; Elagoz, SahandeAim: Estrogen Receptors (ER) and Progesterone Receptors (PR) are known to play a role in breast cancer as both predictive and prognostic markers. However, the influence of Androgen Receptors (AR), a member of the steroid receptor family, remains controversial. In this study, we aimed to determine the relationship between Androgen Receptors (AR) expression and the prognostic factors of breast cancer. Materials and methods: Pathologic specimens of 100 breast cancer patients were stained with an Androgen Receptors (AR) immunohistochemical stain, and the nuclear staining was positive. The intensity and distribution scores of the stainings were summed, and then the total score was calculated. A total score from 0-1 was accepted as negative, while an overall score of 2 and above was positive. The obtained data were evaluated with Chi-square and descriptive statistical methods. A survival analysis was carried out using a Kaplan-Meier test. Findings: The median age of patients was 53 years (range: 17-92 years). Androgen Receptor (AR) positivity was present in 66 (66%) of participants. There were statistically significant relationships between Androgen Receptor (AR) positivity and tumour type (p=0.012), grade (p<0.001), Estrogen Receptor (ER) positivity (p<0.001), Progesterone Receptor positivity (p=0.002), triple negativity of breast cancer (p=0.001), the Ki-67 Index (p=0.007), and the recurrence rate (p=0.043). There were no statistically significant associations between Androgen Receptor positivity and menopausal status, stage, tumour diameter, lymph node involvement, multicentricity, lymphovascular invasion, human epidermal growth factor positivity, or distant metastasis (p>0.05). Conclusion: Androgen receptor may be a good prognostic factor because of its association with low-grade breast cancer), estrogen receptor positivity, progesterone receptor positivity, and low Ki-67 Index values. However, this finding will need to be confirmed by large cohort studies.Öğe Exploring the Prognostic Role of Trop-2, CD47, and CD163 Expression Levels on Survival Outcomes in Patients with Triple-Negative Breast Cancer(MDPI, 2025) Yuceer, Ramazan Oguz; Aydin, Sedanur; Gelir, Iclal; Koc, Tulay; Tuncer, Ersin; Ucar, MahmutBackground: This study evaluated the prognostic impact of Trop-2, CD47, and CD163 expression on clinical outcomes in triple-negative breast cancer (TNBC) and investigated their interactions with tumor progression. Methods: A retrospective cohort of 92 patients with TNBC was analyzed. The expression scores for Trop-2, CD47, and CD163 were categorized as negative/low (0-3 points) or high (4-6 points). The primary endpoint was overall survival (OS). Results: The median age of the cohort was 50 years old. High Trop-2 expression was observed in 55.4% of the patients and was significantly associated with advanced disease stage (p < 0.001). High CD47 expression (44.6%) was correlated with advanced stage (p = 0.044), whereas high CD163 expression (45.7%) was associated with advanced stage (p = 0.021), absence of comorbidities (p = 0.022), and lower pT stage (p = 0.023). Moderate positive correlations were found between Trop-2 and CD47 (p = 0.022), Trop-2 and CD163 (p = 0.037), and CD47 and CD163 (p < 0.001), respectively. Kaplan-Meier survival analysis revealed that patients with low Trop-2 expression exhibited significantly prolonged OS (p = 0.021) and progression-free survival (PFS) (p = 0.026) compared to those with high Trop-2 expression. Univariate and multivariate analyses revealed significant associations between OS and PFS for Trop-2, lymphovascular invasion, and BRCA status. Conclusions: Trop-2 expression is a significant prognostic factor for TNBC and is correlated with worse outcomes. Although CD47 and CD163 showed trends for poorer prognosis, their significance was not confirmed. These findings offer promising prospects for future studies on combined antibody-drug conjugates (ADCs), as they may present opportunities to address multiple resistance mechanisms in the management of TNBC and enhance clinical outcomes.Öğe In-vivo Biocompatibility Evaluation of Different Self-Adhering Composite Resins on Wister Rats(Technical Assoc Photopolymers,Japan, 2021) Saygin, Aysegul; Unal, Murat; Koc, Tulay; Ipek, Irem; Candan, MerveThis study aimed to compare the biocompatibility of two different self-adhering flowable resin composites (SAFCs). 54 polyethylene tubes filled with two different SAFCs [Group V: Vertise Flow (VF), Group F: Fusio Liquid Dentin (FLD), or Group C: empty (Control)] were implanted into the subcutaneous tissue of 18 rats. After 7, 30, and 90 days of implantation, the rats were euthanatized (n=6). The implanted materials were removed with the surrounding tissue for histologic evaluation. The subjects were evaluated in terms of inflammation, fibrous capsule thickness, necrosis, macrophage, and giant cell presence. When the SAFC groups and Group C were compared on days 7, 30, and 90, inflammation, fibrous capsule thickness, and macrophage on the 7th day were statistically significant (p<0.05). Statistically, the giant cell and macrophage criteria were statistically significant on the 7th, 30th, and 90th days in all groups (p<0.05). When the groups were evaluated on different days among themselves, significant differences were observed in terms of local tissue response (p<0.05), while necrosis was found to be statistically insignificant (p>0.05). Biocompatibility was found similar for the SAFCs. These materials used in the study have shown good biocompatibility in rats, and SAFCs can be considered suitable for all restorative treatments.Öğe The effect of platelet-rich plasma on intra-abdominal adhesions in rabbit uterine horn model(Permanyer Publications, 2023) Kurt, Begum; Yildiz, Caglar; Koc, Tulay; Kurt-Ozkaya, Nese; Celikgun, SerkanObjective: This study was carried out to investigate the effect of autologous platelet-rich plasma (PRP) on intra-abdominal adhesion at the cesarean section incision line in the uterus. Material and methods: As experimental animals 16 white New Zealand rabbits, 5-months-old, unmated, were used. Animals were divided into two groups the control group and PRP application group. In each group, a transverse incision was made to the uterus to mimic the cesarean section and sutured. Relaparotomy was performed 21 days after the first operation. Results: When the groups were evaluated in terms of inflammation, there was a significant difference between the two groups. When the groups were evaluated in terms of Mason’s Trichrome staining and fibrosis, There was a significant difference between groups. When the groups were evaluated in terms of vascular endothelial growth factor-1, there was also a significant difference between the groups. In an experimental rabbit uterine horn adhesion model, PRP is effective in preventing post-operative adhesion formation. Conclusions: This result may guide clinical studies using autologous PRP to prevent post-operative adhesion formation after gynecological operations. © 2023 Academia Mexicana de Cirugía. Materials and Methods: Three groups of rats were randomly formed [Group 1: Sham group, group 2: Control group, group 3: Ozone treatment group]. Microscopic and macroscopic scoring were done histopathologically in all groups. Glutathione-s-transferase (GST), superoxide dismutase (SOD), glutathione (GSH), and malondialdehyde (MDA) levels were measured in the colon and liver tissue. Blood cultures were taken for the detection of bacterial translocation. Results: Microscopic damage scores were found as 0.0 (0.0-2.0) in the sham group, 3.0 (3.0-3.0) in the control group, 2.0 (0.0-2.0) in the ozone treatment group (p=0.001). Macroscopic damage scores were found as 0.0 (0.0-1.0) in the sham group, 3.0 (0.0-4.0) in the control group, 0.0 (0.0-1.0) in the ozone treatment group; the scores of ozone treatment and sham groups were found to be statistically different (p=0.004). Compared to the control group, the bacterial translocation in the liver, mesenteric lymph node, portal vein, spleen, and systemic blood was fewer in the ozone treatment group. Statistically significant differences were also observed between the groups' SOD and GST levels in colon tissue and MDA, SOD, and GST levels in liver tissue. Regarding MDA values in the liver tissue of the groups, it was 1.95 +/- 0.43 in Group I, 3.63 +/- 0.81 in Group II, and 1.19 +/- 0.72 U/mg in Group III (p=0.017). When the liver tissue SOD levels of the groups were examined, it was 8.21 +/- 0.76 U/mg in Group I, 4.57 +/- 0.58 U/mg in Group II, and 8.62 +/- 1.13 U/mg in Group III (p=0.029). When GST values in liver tissue belonging to the groups were examined, 0.35 +/- 0.16 in Group 1, 0.23 +/- 0.03 in Group II, 0.49 +/- 0.13 U/mg in Group III (p=0.036). Conclusion: Rectal ozone application plays a role in increasing the organism's antioxidant activity and has an effective role in increasing the enzyme activities of antioxidant defenses. In addition, rectal ozone application has a positive effect on wound healing at a histopathological level and reduces bacterial translocation in various tissues.Öğe The effect of smoking and electronic cigarettes on rat testicles(Elsevier Espana Slu, 2023) Saygin, Huseyin; Korgali, Esat; Koc, Tulay; Dogan, KubraObjective: After the negative effects of smoking on public health were proven, smoking cessation campaigns were initiated by health ministries and non-governmental organizations. Many drugs have been tried to reduce the addiction to smoking and the nicotine contained in it. Recently, e-cigarettes (EC) are widely used for smoking cessation efforts, although the effects and possible harms are not fully known. In our study, we planned to show the effect of cigarette and EC smoke on the male urogenital system. Methods: Adult male wistar rats were exposed to cigarette and EC smoke in a specially designed glass bell jar. Urine cotinine levels, testicular weights, gonadosomatic index, sperm count and sperm motility, testicular histology, and biochemical findings were compared with the control group. Results: In some rats in the cigarette and EC group, the seminiferous tubules were disorganized, and the germ cells and Sertoli cells were separated and shed. Stopped germ cell separation, cavity formation, necrosis, fibrosis, and atrophy were observed in severe cases. Higher PCO levels were found in the cigarette group compared to controls. Tissue homogenates levels of LPO were higher in both EC and cigarette groups compared to controls. No significant differences were observed between groups in terms of sperm motility and sperm count. Conclusion: Cigarette and EC liquid can increase oxidative stress as well as cause morphological changes in the testicle.
