    Diabetes Mellitus Patients with Fibromyalgia have a Higher Risk of Cardiovascular Disease

    Aim: The primary aim of this study is to investigate the effect of the presence of Fibromyalgia(FM) on cardiovascular risk and cardiopulmonary capacity in diabetes mellitus (DM) patients. In addition, the prevalence of FM in DM patients was investigated according to the American College of Rheumatology (ACR) 2016 criteria. Material and Methods: The study included 188 premenopausal female patients with Type 2 DM. The physical activity level of the study participants was assessed with the International Physical Activity Questionnaire - short form. The cardiopulmonary capacity was evaluated with the 6-minute Walk Test. The cardiovascular risks were evaluated with the Framingham risk score. Participants were examined for the presence of FM according to the ACR 2016 criteria. Results: 63 (33.5%) participants met the ACR 2016 FM diagnostic criteria. DM patients with FM have significantly lower physical activity levels (p=0.004) and cardiopulmonary capacity (p=0.009), and they also have significantly higher cardiovascular risks (p=0.02) than DM patients without FM. Conclusion: FM is seen in approximately one-third of DM patients, and the presence of FM reduces cardiopulmonary capacity and increases cardiovascular risk. In the management of patients diagnosed with DM, the presence of FM should be investigated, and treatment should be applied with pharmacological and non-pharmacological methods to reduce the contribution to cardiovascular risk.

    Inter-observer agreement of the musculoskeletal system disability levels ofindividuals presenting at disability health boards

    The classification and scoring of disability in Turkey are done within the framework of the “Regulation on Disability Assessment for Adults”. However, evaluation of\rdisabled individuals by different physicians despite having the same disability may lead to conflicting results. This study aims to investigate the inter-observer agreement\rof physiatrists and orthopedists in the evaluation of patients who applied to the Disability Health Board because of musculoskeletal system pathologies. Ten people with\rmusculoskeletal problems and four physicians, including two physical therapists and two orthopedists, were included in this study. The patients were evaluated by the\rphysicians according to the provisions of the “Regulations for the Disability Assessment of Adults” in random order in separate rooms on the same day. The mean disability\rpoints were found to be 19.4±14.8 for physiatrist 1 (PMR1), 16.5±13.1 for PMR2, 17.9 ±14.9 for orthopedist 1 (O1), and 18.7±15.9 for O2. No statistically significant difference was determined between the 4 physicians in respect of the points given (p=0.85). The agreement between all 4 physicians was found to be 0.85 (95% CI:0.68-0.95).\rIn the paired analyses, the ICC ranged from 0.73 to 0.96, with the lowest agreement between PMR2-O2, and the highest agreement between O1-O2. In conclusion, the\rresults of this study showed that there was not an absolute agreement between physiatrists and orthopedists because of the complex structure in determining the disability\rrate of the musculoskeletal system, the fact that patient compliance is important, and that the approach of the physician can be different. For physicians participating in\rthe Health Board to be able to provide standardization, there is a need for theoretical and patient-based training, the appropriate time to be allocated for patient evaluation,\rand the open points in the items of the regulation should be eliminated.

    Neuropathic Pain Component in Patients with Ankylosing Spondylitis and the Relationship of Neuropathic Pain and Disease Activity Parameters: A Cross-Sectional Study

    Objective: It is known that neuropathic pain frequently accompanies rheumatological diseases. In this study, neuropathic pain in Ankylosing Spondylitis(AS) and its relationship with disease activity were investigated. Methods: Forty patients with AS were included. Laboratory data and disease status parameters were recorded. Neuropathic pain questionnaires were administered. Electrophysiological examination was performed on all patients. The relationship between neuropathic pain and disease activity parameters was investigated. Results: According to the Pain Detect and LANSS questionnaire results, the rate of neuropathic pain was 57.5% and 42.5%. ASQoL, BASDAI, and ASDASESH parameters are statistically significantly higher in the group with neuropathic pain according to the PainDetect (p:0.018, p:0.04, p:0.028). MASES, ASQoL, BASDAI, BASFI, and ASDAS-ESH parameters are statistically significantly higher in the group with neuropathic pain according to the LANSS (p:0.004, p:0.005, p: 0.001, p:0.005, p:0.02). Disease activity is higher in patients with neuropathic pain for both scales. Peripheral neuropathy is detected in nine patients. There is a positive correlation between disease activity parameters and neuropathic pain scales. A strong positive correlation was detected between ASQoL and BASDAI parameters and the Pain Detect questionnaire (r:0.533, r:0.606). Conclusions: The majority of patients with AS have a neuropathic pain. This condition is associated with high disease activity and adversely affects the patient's quality of life.

    Sarkopenisi Olan Diz Osteoartritli Yaşlılarda Dizliğin Diz Eklem Propriyosepsiyon Duyusuna ve Statik Dengeye Etkisi

    Amaç: Sarkopenik yaşlılarda ekstremite hareketlerinde farkındalığın art ması ve dengenin iyileştirilmesi, düşmelerin ve düşme ile ilişkili fiziksel, ruhsal ya da sosyal problemlerin önüne geçebilir. Bu araştırmada amaç, sarkopenisi olan yaşlı diz osteoartritli bireylerde elastik neopren dizlik kullanımının diz eklem propriyosepsiyon duyusuna ve statik dengeye etkisini araştırmaktır. Gereç ve Yöntemler: Uluslararası Fiziksel Tıp ve Rehabilitasyon Derneği Sarkopeni Ça lışma Grubu önerileri doğrultusunda el kavrama gücü, sandalyeden kalkma testi ve sonografik uyluk düzeltme oranına göre sarkopeni tanısı alan 61 yaşlı bilate ral diz osteoartriti olan kadın hastanın dâhil edildiği bu kesitsel araştırmada, ka tılımcıların elastik neopren dizlikli ve dizliksiz statik denge ölçümü ve diz eklem propriyosepsiyon duyusu ölçümleri yapıldı. Diz eklem propriyosepsiyon duyusu, izokinetik dinamometre ile dominant ekstremiteden 45° fleksiyon açısı için mo difiye bir reprodüksiyon yöntemi kullanılarak ölçüldü. Statik denge, bilgisayarlı denge sistemi kullanılarak genel stabilite indeksi, ön-arka stabilite indeksi ve me dial-lateral stabilite indekslerine göre değerlendirildi. Dizliksiz ve dizlik giyile rek yapılan ölçümlerinin karşılaştırılmasında bağımlı gruplar t-testi kullanıldı. Bulgular: Ortalama yaşı 69,7 olan katılımcıların olduğu bu araştırmada, 6±1,2 olan diz fleksiyon sapma açısı dizlik giyilmesi sonrası 5,6±1,3’e istatistiksel an lamlı olarak geriledi (p=0,02). Sırasıyla 7,45±0,43, 6,78±0,29 ve 6,55±0,33 olan genel stabilite indeksi, ön-arka stabilite indeksi ve medial-lateral stabilite indeksi skorları dizlikli ölçümde sırasıyla 7,38±0,51, 6,64±0,43, 6,53±0,25 bulundu ancak değişim istatistiksel olarak anlamlı değildi (sırasıyla p=0,55, p=0,47, p=0,15). Sonuç: Elastik neopren dizlik kullanımı yaşlı sarkopenisi olan diz osteoartritli hastalarında diz eklem propriyoseptif duyusu keskinliğini artırır ancak statik den geye etkisi yoktur. Dizlik kullanımı, diz ekleminin kinestetik farkındalığını artı rarak olası yaralanmaların önüne geçilebilir.

    Suicidal Ideation and Behaviors In Mothers Of Children with Cerebral Palsy

    Objective: Long-lasting care process can result in the withdrawal of the mother with children diagnosed with cerebral palsy (CP) from the social environment and cultural activities while leading also to physical exhaustion. The present study evaluates the levels of depression and anxiety, suicidal ideation, and behaviors in mothers with children diagnosed with CP.Methods: The study included 57 healthy mothers with children aged 2–16 years who have been diagnosed with CP and 41 healthy mothers with healthy children as a control group. The Hospital Anxiety and Depression Scale (HADS), the Scale for Suicidal Ideation (SSI), and the Suicidal Behavior Scale (SBS) were administered to all participants.Results: The sociodemographic status of the mothers of children with CP was similar to that of the mothers with healthy children (p>0.05). The HADS, SSI, and SBS scores were found to be higher in the mothers with children with a CP diagnosis than in mothers with healthy children (p<0.05).Conclusion: The risk of attempted suicide was higher in mothers of children with CP than in those of healthy children. Accordingly, the observation of the psychological status of mothers with children with CP should be considered important.

    The association between prognosis and sarcopenia assessed by psoas muscle measurements in elderly male patients with covid-19

    Introduction: Sarcopenia, which is defined as a generalized loss of skeletal muscle mass and function, is an important clinical condition, especially among elderly patients with comorbidities. This study assesses the association between prognosis and sarcopenia assessed by psoas muscle measurements in elderly male patients with coronavirus disease 2019. Materials and Methods: This study included 130 consecutive elderly male patients hospitalized with coronavirus disease 2019. The participants were followed until their time of discharge or death. The psoas muscle cross-sectional area, psoas density, and psoas muscle index measurements were performed based on a single-axial computed tomography section at the L2 vertebral corpus. The outcome measurements of the patients were determined as intensive care unit admission and mortality. Results: During the clinical course of these patients, 29.2% (38) were transferred to the intensive care unit, and 13.1% (17) died. The interclass correlation coefficients for the psoas muscle cross-sectional area and density were 0.91 and 0.93, respectively, between observers. Low psoas muscle index is significantly associated with mortality (p = 0.001), and the probability of mortality increases approximately 5–30 times as the psoas muscle index decreases. No factors were identified as being significantly associated with intensive care unit admission. Conclusion: In elderly male patients who are hospitalized due to coronavirus disease 2019, an assessment of the psoas muscle may provide additional information on the prognosis of the disease. © 2021, Geriatrics Society.     The Effect of Abdominal Muscle Activity on Postural Control in Patients with Postmenopausal Osteoporosis

    Objective: The trunk muscles are a component of the neuromuscular subsystem of postural control due to their location in the center of the kinetic chain. The aim of this study was to investigate the effect of abdominal muscle activity on postural control in patients with postmenopausal osteoporosis. Materials and Methods: Fifty-six postmenopausal osteoporosis patients with a fear of falling were included. Postural stability of the participants was evaluated with timed up and go test (TUG), Berg balance scale (BBS), and static balance index (SBI). Abdominal muscle activities were evaluated using a pressure biofeedback unit involving the performance of an abdominal drawing-in maneuver and posterior pelvic tilt movement. According to the postural stability test scores, the participants were divided into subgroups as high fall risk and low fall risk. Results: Twenty (35.7%) patients according to the TUG, 24 (42.8%) patients according to BBS, 18 (32.1%) patients according to SBI had a high risk of falling. In all subgroups analyzes, patients with a high risk of falling had more fall history (p=0.007, p<0.001, p<0.001, respectively), less pelvic tilt movement activity (p<0.001, both), and abdominal draw-in muscle activity (p<0.001, p=0.009, p<0.001, respectively). Conclusion: Patients with postmenopausal osteoporosis who have a high risk of falling have low abdominal muscle activities. While rehabilitating these patients might be performed core stabilization exercises that can increase the activation of the abdominal muscle activities.

    The Relationship Between Dual-Task Performance and Functional Independence in Stroke Patients

    Purpose: Dual-task performance have a role in independent motor functions such as gait and balance. Although walking and balance parameters are important in functional independence, the level of independence is a complex condition affected by many factors. The aim of this cross-sectional study is to investigate the relationship between dual-task performance and functional independence levels in stroke patients. Material and Methods: 26 hemiplegic patients were included in this research in a tertiary healthcare institution. The participants’ independence levels were evaluated by the functional independence mesasure(FIM). Single task performance, motor dual-task performance, and cognitive dual-task performance were evaluated by calculating walking time in a 10-meter walking test. The relationship between dual-task interference and functional independence level was examined by Spearman correlation analysis. Results: The motor dual-task interference was found as 0.7 (0.0-0.26) and the cognitive dual-task interference was found as 0.19 (0.05-0.38). There was a negative, moderate, and statistically significant correlation between the FIM score and the motor dual-task interference (r = -0.48, p = 0.01). However, no significant correlation was detected between FIM score and cognitive dual-task interference (p = 0.16). Conclusion: The level of functional independence in stroke patients is associated with motor dual-task interference. Rehabilitation programs that improve dual-task performance can increase the functional independence levels of stroke patients.

    The role of ultrasonographic synovial assessment in rheumatoid arthritis patients with concomitant fibromyalgia

    Objectives: This study aimed to compare the prevalence and musculoskeletal ultrasonography (US) findings of rheumatoid arthritis (RA) patients with concomitant fibromyalgia (FM) according to the 1990 American College of Rheumatology (ACR) FM classification criteria or the 2016 ACR FM diagnostic criteria. Patients and methods: This cross-sectional study included 63 patients (17 males, 46 females; mean age: 48.2 +/- 7.1 years; range, 18 to 62 years) with RA. Medical history and laboratory data were obtained from electronic records. Clinical examination, composite disease activity measures, functional status, and the German 7-joint ultrasound score were assessed to evaluate disease activity and synovial inflammation. The patients were divided into three groups: patients who met only the 2016 ACR criteria, patients who met only the 1990 ACR criteria, and patients who met both criteria. Results: In patients with RA, concomitant FM prevalence was 34.9% according to the 2016 ACR FM diagnostic criteria versus 23.8% according to the 1990 ACR FM classification criteria. Rheumatoid arthritis patients with FM had high tender joint count and disease activity scores, while musculoskeletal US findings were similar. Patients who met only the 2016 ACR FM diagnostic criteria had significantly higher gray-scale US and power Doppler US synovitis scores than patients who satisfied only ACR 1990 FM classification criteria (p=0.03 and p=0.02, respectively). Conclusion: Synovial inflammation is a prominent sign in RA patients diagnosed with FM according to the 2016 ACR FM diagnostic criteria. The higher disease activity seen in the presence of FM in RA patients is associated with FM rather than synovitis.

    What is the role of sensitization in carpal tunnel syndrome where pain impacts functional capacity?

    Aims: Expression of carpal tunnel syndrome (CTS) caused by the entrapped median nerve includes pain, paraesthesia and numbness. Extra median spread of pain can furthermore be seen as a clinical requirement defined by neuro inflammation. Central sensitization (CS) constructs a similar mechanism. This study aims to evaluate sensitization in patients diagnosed with CTS. Methods: A total number of 152 patients diagnosed with CTS were evaluated, prospectively. Patients data such as gender, age, body mass index (BMI), disease duration, pain severity (NRS), painDETECT questionnaire, Boston CTS symptom severity scale (SSS) and functional status scale (FSS), CS scale, electroneuromyography results have been collected by the author and then the patients are divided into three groups. Results: Regarding the age, BMI and CS rate, there was no statistical difference between the three patient groups (p>0.05). However, a statistically significant difference was found between these groups in disease duration, day-time and night-time NRS, Boston SSS, FSS, and pain DETECT scores (p0.05).
