    Lenfödem Yönetiminde Egzersizin Rolü: Bibliyometrik Bir Analiz

    Lenfödem yaşam boyu süren kronik bir sağlık sorunudur ve egzersiz yaklaşımları lenfödem tedavisinde altın standartlardan biri olarak kabul edilmektedir. Lenfödemde egzersiz yaklaşımları hakkında çok sayıda makale yayınlanmıştır. Bu çalışmada, lenfödemde egzersiz ile ilgili çalışmaların bibliyometrik bir analizini yapmayı amaçladık. Bu amaçla 24 Ekim 2023 tarihinde Scopus veri tabanında “lymphedema and exercise” anahtar kelimeleri kullanılarak bir arama yapıldı ve 869 doküman incelendi. Veriler ilgili arama motorundan indirildi. Bu veriler Vosviewer ve Biblioshiny paket programları kullanılarak analiz edildi. Yayın tarihleri 1957 ile 2023 arasında değişmektedir. Analiz sonuçlarına göre, makale yayınları açısından Amerika Birleşik Devletleri önde gelirken (n=979), uluslararası iş birliği açısından Avusturya liderdir (n=401). Lenfödem ve egzersiz anahtar kelimeleri kullanılarak en çok atıf alan yayın Campbell ve arkadaşlarının (2019) “Exercise Guidelines for Cancer Survivors: Consensus Statement from International Multidisciplinary Roundtable” başlıklı makalesi, 1101 atıfla ‘Medicine and Science in Sports and Exercise’ dergisinde yayımlanmıştır. “Lymphatic Research and Biology”, ‘Lymphology’ ve ‘Supportive Care in Cancer’ bu alanda en çok atıf alan ilk üç dergidir. Özellikle Pennsylvania Üniversitesi, bu alana verimli araştırma çıktılarıyla katkıda bulunan en üretken kurum olarak öne çıkıyor. En üretken yazar 29 makale ile Kathryn H. Schmitz’dir. En sık kullanılan anahtar kelimeler arasında “lenfödem”, “meme kanseri” ve “lenfödem, yaşam kalitesi” yer almaktadır. Bu çalışmanın bulguları sadece lenfödem ve egzersizle ilgilenen araştırmacılar ve uygulayıcılar için değil, aynı zamanda pratik uygulamalar ve gelecekteki potansiyel gelişmelerle ilgilenen herkes için değerlidir. Sonuç olarak, bu kapsamlı bilgi rehberi gelecekteki çalışmalar için önemli bir kaynak ve yol haritası görevi görebilir.

    Reliability and criterion validity of the Turkish version of the Recent Physical Activity Questionnaire

    Background: This study aims to determine reliability and criterion validity of the Turkish version of the Recent Physical Activity Questionnaire (RPAQ). Materials and methods: The study included 146 individuals aged 18-45 years. Translation and cultural adaptation of the questionnaire was performed with regard to an appropriate procedure. Test-retest reliability was analyzed in terms of intraclass correlation coefficient (ICC). For assessment of validity of the questionnaire, the criterion validity method was used. Criterion validity was evaluated using a pedometer, which is an objective measurement tool. The correlation between the total number of steps recorded during a week and RPAQ scores was examined. The validity was evaluated using Spearman correlation analysis. Results: The ICC value was 0.96 for total PAEE (physical activity energy expenditure) and between 0.77 and 0.97 for home, work, recreation, and transportation PAEE. The duration of intensity levels varied between 0.80 and 0.97. Test-retest reliability of RPAQ was high. A moderate correlation (r = 0.472) was found between the total value of RPAQ and the number of steps, and a weak- to-moderate correlation (r = 0.210-0.442) was found for other dimensions. No correlation was found with home activities. Conclusion: The Turkish version of RPAQ is valid and reliable. RPAQ can be used to assess the physical activity level of the Turkish population.

    The effect of two different types of electric current on experimental pain threshold in healthy individuals: A double blind, randomized controlled trial

    Objective: Physiotherapists often resort to electrophysical agents for pain management.The aim of this study is to determine the effect of acupuncture-like Transcutaneous Electrical Nerve Stimulation (TENS) and Neuromuscular Electrical Stimulation (NMES) on experimental pressure pain threshold (PPT) in healthy individuals.Method: Sixty one healthy participants (44 females, 17 males) aged 18-21 years were included in the study.The participants were divided into acupuncture-like TENS and NMES groups. The application was performed for 20 minutes from the right upper limb. The PPT changes of the participants were evaluated from the thenar part of both hands with an algometer before the current application, immediately after and 60 minutes after.Results: There was no difference in the PPT levels of the right and left extremities of the participants in both groups before the application (p>0.05). In both groups and intergroup comparisons, there was no difference in PPT levels measured from the right and left extremities of participants before the current application, immediately after and 60 minutes after the current application (p>0.05).Conclusions: In this study, it was concluded that acupuncture-like TENS and NMES had no effect on experimental PPT and therefore, the clinical use of currents should be questioned.

    WHOLE-BODY VIBRATION EFFECT ON MUSCLE ACTIVATIONS: WHICH ONE IS THE MOST EFFECTIVE, LOW FREQUENCY OR HIGH FREQUENCY?

    Purpose: Whole Body Vibration (WBV) is a practice that passively applies mechanical oscillations to an individual from a support surface. The tonic vibration reflex response depends on the vibration localization, frequency, amplitude, and initial length of the muscle, but there is no consensus on what the optimal frequency should be. This study was conducted to examine the activation differences of lower extremity muscles at low and high frequencies during squat exercise on WBV. Methods: This study involved 16 healthy individuals (Age = 23.66 +/- 2.33 years, Body Mass Index = 22.59 +/- 3.86 kg/m2). WBV application was performed on a vertical vibration platform (GLOBUS Physioplate (R)). Participants performed static half-squats on WBV for 20 seconds under vibrating (20 Hz and 60 Hz; 2-3 mm amplitude) conditions. An 8-channel Electromyography (EMG) Noraxon MiniDTS system was used to measure the activation of the Gluteus Medius (GMed), Gluteus Maximus (GMax), Vastus Lateralis (VL), and Vastus Medialis (VM) muscles. Results: It was observed that there was a difference between the two frequencies for the activation of the VM, VL, and GMed muscles (p = 0.004, 0.001, 0.002, respectively). Vibration frequencies of GMed, VL, and VM muscle activities at high frequency were increased compared to low frequency. GMax did not show any statistically significant change between the two vibration conditions (p=0.013). Conclusions: Physiotherapists and trainers should prefer high frequencies in WBV applications, especially when they need to improve the neuromuscular response in the quadriceps and gluteus medius muscles.
