dc.description.abstractChronic Renal Failure is considered as an important health problem due to permanent kidney damage, symptomatic treatment and continuity of treatment protocols, and requires a holistic perspective when planning care. In this process, nurses have important roles in the treatment, follow-up and care of the disease together with all healthcare professionals. It is important to develop approaches that ensure standardization of care for successful service delivery of nurses in chronic disease management. The use of nursing models for care approaches is known to be guiding. Many important nursing theories and models are recommended to be used in nursing practice both in the nursing management of chronic diseases and for the development and advancement of professional nursing. In this article, the nursing process of a patient with chronic renal failure was created using the Neuman Systems Model. Data were collected with a diagnostic form in accordance with the model, and a care plan including primary, secondary and tertiary prevention levels was created for the patient in line with the formulation of the problem. This case showed that the use of Neuman's Systems Model was useful and effective in the implementation of the nursing process of an individual with chronic renal failure.
dc.description.abstractKronik Böbrek Yetmezliği kalıcı böbrek hasarının olması, tedavinin semptomatik olarak sürdürülmesi ve tedavi protokollerinin süreklilik gerektirmesi nedeniyle önemli bir sağlık sorunu olarak kabul edilmekte ve bakım planlanırken bütüncül bir bakış açısı gerektirmektedir. Bu süreçte hastalığın tedavi, izlem ve bakımında tüm sağlık çalışanları ile birlikte hemşirelere önemli roller düşmektedir. Kronik hastalık yönetiminde hemşirelerin başarılı hizmet sunumu için bakımın standartlaşmasını sağlayan yaklaşımlar geliştirilmesi önemlidir. Bakım yaklaşımları için hemşirelik modellerinin kullanımının yol gösterici olduğu bilinmektedir. Birçok önemli hemşirelik kuram ve modelinin, hem kronik hastalıkların hemşirelik yönetiminde hem de profesyonel hemşireliğin gelişim ve ilerlemesi için hemşirelik uygulamalarında kullanılması önerilmektedir. Bu makalede, Neuman Sistemler Modeli kullanılarak kronik böbrek yetmezliği olan bir hastanın hemşirelik süreci oluşturulmuştur. Modele uygun tanılama formu ile veriler toplanmış, problemin formulasyonu doğrultusunda hasta için birincil, ikincil, üçüncül korunma düzeylerini içeren bakım planı oluşturulmuştur. Bu olgu, kronik böbrek yetmezliği olan bir bireyin hemşirelik sürecinin uygulanmasında Neuman’ın Sistemler Modeli’nin kullanımının sistematik bir bakım planı için yararlı ve etkili olduğunu göstermiştir.
