Uzun süre topikal antiglokomatöz ilaç kullanımının trabekülektomi başarısı ve konjonktival yüzeye etkisi

Abstract

Bu çalışmada uzun süre topikal antiglokomatöz ilaç kullanımının tra¬bekülektomi başarısı ve oküler yüzeye olan etkileri araştırıldı. Retros¬pektif olarak yapılan bu çalışmaya, primer açık açılı glokom (PAAG) tanısı ile trabekülektomi uygulanan ve en az 1 yıl takip edilen 43 has¬ta alındı. Trabekülektomiden ortalama 36.6±15.8 ay sonra, hastaların trabekülektomi başarısı, gözyaşı parçalanma zamanı(FPZ), bazal Schirmer testi ve konjonktiva impresyon sitoloji testleri yapıldı. Has¬talar preoperatif topikal antiglokomatöz ilaç kullanım süresine göre 2 gruba ayrıldı. 3 yıldan daha kısa süre İlaç kullanan hastalar A grubuna, 3 yıldan daha uzun süre ilaç kullanan hastalar B grubuna alındı. A gru¬bunda başarı oranı % 96 iken B grubunda %66.6 idi (p<0.05). A gru¬bunda gözyaşı kırılma zamanı ll.Otl.l sn, bazal Schirmer testi 13.5±3.3 mm iken aynı değerler B grubunda sırasıyla 6.1±2.4 sn ve 6.7±3.0 mm idi (p<0.05). Konjonktiva impresyon sitolojisi skoru A gru¬bunda 15 (%60) hastada grade 0. 9 (%36) hastada grade 1, 1 (%4) has¬tada grade 2'de iken, B grubunda grade O'da hasta yok, grade l'de 1 (%5.5) hasta, grade 2de 7 (%39) ve grade 3de 10 (%55.5) hasta vardı (p<0.05). Bu sonuçlarla 3 yıl ve daha uzun süre topikal antiglokomatöz ilaç kullanımının tabekülektoml başarısı üzerine ve konjonktiva yü¬zeyine olumsuz yönde etkileyebileceği sonucuna varıldı.

Effects of Topical Antiglaucoma Medication on the Conjunctiva Surface and Out Come of Trabeculectomy In this study, the effect of long-term topical antiglauco-matous medication on the ocular surface and success rate of trabeculectomy have been investigated. This ret&not;rospective investigation covered 43 cases diagnosed as primary open angle glaucoma (POAG). subjected to tra&not;beculectomy and followed up for at least one year. About 36.6&plusmn;15.8 months after trabeculectomy, the suc&not;cess rate of trabeculectomy, break up time, basal schir&not;mer test and conjunctiva impression cytology were eva&not;luated. The patients were classified in two groups. Group A using antiglaucomatous medication' for less than 3 years and Group B for more than 3 years. While success rate of trabeculectomy 96% in group A, it was observed to be success rate of trabeculectomy 66.6% in group B (p&lt;0.05). The break up time and basal Schirmer test in group A were Il.Otl.l sec and 13.5&plusmn;3.3 mm , respectively and those in group B were 6.1&plusmn;2.4 sec and 6.7&plusmn;3.0 mm, respectively. While the conjunctiva im&not;pression cytology scores in group A were grade 0 in 15 patients (60%), grade 1 in 9 patients (36%), and grade 2 in 1 patient (4%), in group B grade 0 was not seen in any patient, grade 1 was observed in 1 patient (5.5%), grade 2 in 7 patient (39%) and grade 3 in 10 patients (55.5%) (p&lt;0.05). Considering these findings, it was concluded that admi&not;nistration of topical antiglaucomatous medication for 3 years or more may bear adverse effects on the conjunc&not;tiva surface and success rate of trabeculectomy.
