Alerjik rinitli hastalarda prick testi ve total IgE sonuçlarının değerlendirilmesi

Abstract

Amaç: Öykü ve fizik muayene ile alerjik rinit ön tanısı konan hastalarda atopik yapı, klinik ve laboratuvar verileri arasındaki ilişki ve bunların tanısal değerleri değerlendirildi.Hastalar ve Yöntemler: Nazal şikayetleri ve alerji anamnezi olan ve prick testlerle en az bir alerjene karşı duyarlı olduğu saptanan 64 hasta incelendi. Herhangi bir alerji öyküsü olmayan 20 kişilik kontrol grubu oluşturuldu. Total IgE düzeyleri ölçüldü ve prick testi uygulandı. Prick testi sonuçlarına göre hastalar atopik ve nonatopik olarak iki gruba ayrıldı. Prick testi sonuçları ile total IgE değerleri karşılaştırıldı. Bulgular: Atopiklik açısından gruplar arasında anlamlı fark saptandı (p<0.05). En yüksek atopiklik ev tozlarında, mantarlarda ve yabani otlarda gözlendi. Total IgE düzeyi hasta grubunda 148.06±20.17 IU/ml, kontrol grubunda 58.45±14.4 IU/ml idi (p<0.05). Alerjik olunan madde sayısı ile total IgE değerleri arasında anlamlı ilişki yoktu. Sonuç: Alerjik rinitte total IgE tayini tanıyı destekleyici bir yöntemdir.

Objectives: The relationship between atopic structure, clinical and laboratory findings and their diagnostic value were evaluated in patients with an initial diagnosis of allergic rhinitis. Patients and Methods: Sixty-four patients with nasal complaints, a history of allergy, and prick test sensitivity to at least one allergen were studied. A control group of 20 subjects without a history of allergy was included. Total IgE levels were measured and prick tests were performed. Patients were divided into two groups as atopic and nonatopic according to prick test results. Prick test results and total IgE values were compared. Results: Significant differences were found with regard to atopy between the two groups (p&lt;0.05). The highest levels of atopic reactions were against home dust, fungus and wild grass. Mean total IgE levels were significantly different (148.06&plusmn;20.17 vs 58.45&plusmn;14.4 U/ ml) (p&lt;0.05). The number of atopic allergens and total IgE values did not differ significantly (p&gt;0.05). Conclusion: Total IgE levels may be instrumental in the diagnosis of allergic rhinitis.
