Proparakain ve oksibuprokainin oküler yüzeye anestetik ve yan etkileri

Abstract

AMAÇ: Bu çalışmada topikal anestezik olarak proparakain ve oksibuprokainin anestezik etkinliği ile yan etkileri değerlendirildi. YÖNTEM: Çalışmaya oküler patolojisi olmayan rastgele seçilmiş 140 hasta alındı. Seçilen hastalara ayrı zamanda tek doz topikal proparakain ve oksibuprokain uygulandıktan sonra, Goldmann applanasyon tonometresi ile göziçi basıncı ölçüldü. Testi uygulayan kişi tarafından anestezinin yeterliliği, hastanın ilaç damlatılmasına karşı gösterdiği reaksiyon, ilacın neden olduğu konjonktival hiperemi ile hasta tarafından ifade edilen tonometriye karşı duyarlılık ve ilacın neden olduğu yanma değerlendirilerek skorlandı. BULGULAR: Proparakain uygulanan hastalarda oksibuprakaine göre hem daha mükemmel seviyede anestezi sağlanırken hemde ilaca bağlı yan etkilerin daha az olduğu saptandı. Proparakain uygulanan 125(%89.3) hastada mükemmel seviyede anestezi sağlanırken(p<0.01, P=0.002), 105(%75) hastada ilaç damlatılması bağlı hiçbir reaksiyon görülmedi(p<0.001, P=0.0001), ayrıca 126(%90) hastada konjonktival hiperemi saptanmadı (p<0.001, P=0.0001). Aynı veriler oksibuprokain uygulanan hastalarda sırasıyla 110(%78.6), 23(%16.4) ve 70(%50) idi. Proparakain uygulanan hiçbir hastada şiddetli seviyede yanma ve tonometriye karşı duyarlılık saptanmazken(p<0.001, P=0.0001), oksibuprokain uygulanan 28(%20) hastada şiddetli yanma ve 23(%16.4) hastada şiddetli seviyede duyarlılık saptandı.SONUÇ: Bu bulgularla göziçi basıncını ölçmek için topikal anestetik olarak proparakain kullanılması hem daha yeterli anestezi sağlandığı hem de ilaç uygulamasına bağlı yan etkilerin daha az olduğu sonucuna varıldı.

PURPOSE: This study was designed to evaluate the effects and adverse effects of topical anesthetic proparacaine and oxibuprocain. METHODS: 140 patients who had no eye disease were selected randomly. After selected patients, a single dose of either 0.5% proparacaine or 0.4% oxibuprocain was applied and, intraocular pressure was measured by Goldmann applanation tonometry. Examiners graded the adequacy of anesthesia, patient tolerance of the drop and induced hyperemia by anesthetic, while the subjects reported on the sting of the drop, awareness of the tonometer after procedure. RESULTS: More satisfactory anesthesia and less amount of side effects were obtained with proparacaine compared to oxibuprocain. While in 125(89.3%) the patients given proparacaine a perfect level of anesthesia was provided(p&lt;0.01, P=0.002), in 105(75%) patients no reaction was seen due to the drop(p&lt;0.001, P=0.0001) and in 126(90%) patients no conjunctival hyperemia was found(p&lt;0.001, P=0.0001), The same data was seen in the patients given oxibuprocain as 110 (78.6%), 23 (16.4%), and 70 (50%) respectively. While no marked sting of drop and awareness of tonometer was found in any patients given proparacaine (p&lt;0.001, P=0.0001), in 28(20%) patients given oksibuprokain marked sting of drop and in 23(16.4%) patients marked sensitivity was determined. CONCLUSION: Finally, we concluded that the use of proparacaine as a topical anesthetic to measure intraocular pressure would provide more satisfactory anesthesia and less side effects than the use of oxibuprocain.
