Hiperbilirübinemili yenidoğanlarda fototerapi öncesi ve sonrası eritrosit glutatyon-S-transferaz aktiviteleri
Abstract
Glutatyon-S-transferaz (GST) enzimi karsinojenler dahil birçok ekzojen ya da endojen kaynaklı organik bileşikleri glutatyon ile konjugasyonunu katalizleyen çok işlevli kompleks bir enzim grubudur. Sunduğumuz çalışmada fototerapide uygulanan beyaz flöresan ışığın eritrosit GST enzim aktivitesine etkisi araştırıldı. Bu amaçla 20 hiperbilirübinemili hastada fototerapi öncesi ve fototerapi sonrası (24 saatte) eritrosit GST aktiviteleri ölçüldü. Fototerapi öncesi ortalama eritrosit GST aktivitesi 29.47 ± 16.04 nmol/gr/Hb, fototerapi sonrası eritrosit GST aktivitesi ise 33.77 ± 15.91 nmol/gr/Hb olarak saptandı. Fototerapi sonrası ortalama eritrosit GST aktivitesi, fototerapi öncesine göre anlamlı şekilde yüksekti (p<0.05). Sonuç olarak fototerapi sonrası GST aktivite artışı yenidoğanda detoksifikasyon metabolizmasının çalıştığını ve fototerapiden gelecek olumsuz etkileri de ortadan kaldırabileceğini göstermektedir. Glutathione S-transferases (GSTs) are a multifunctional, complicated enzyme group that catalyzes the conjugation of glutathione with exogenous or endogenous organic compounds. Phototherapy, generally used for the therapy of newborns with hyperbilirubinemia, has toxic effects on several systems. The present study investigated the effects of white fluorescence light used for phototherapy on erythrocyte GST enzyme activity. Twenty patients with hyperbilirubinemia were exposed to white fluorescence light. Erythrocyte GST activities light and before phototherapy and 24 hours after phototherapy were measured. Before phototherapy mean erythrocyte GST activity was 29.47 ±16.04 nmol/gr/Hb whereas the after phototherapy mean erythrocyte GST activity was 33.77 ±15.91 nmol/gr/Hb. After phototherapy mean GST activity was found to be significantly (p<0.05) higher than the before phototherapy mean GST activity. In conclusion, increased after phototherapy GST activity may indicate the presence of detoxification metabolism in newborns. This detoxification mechanism may also eliminate the adverse effects of phototherapy.
