Alt ekstremite iskemi reperfüzyona bağlı gelişen akciğer hasarında askorbik asidin etkisi
Abstract
Amaç: Bu çalışmada, alt ekstremitelerdeki geçici iskemi reperfüzyon (İ/R) hasarına bağlı akciğerlerde oluşan patolojik değişiklikler ve bunları önlemede askorbik asidin koruyucu etkisi araştırıldı. Materyal ve Metod: Ratlar kontrol grubu (Grup 1), iskemi reperfüzyon grubu (Grup 2) ve askorbik asit verilen çalışma grubu (Grup 3) olmak üzere üç gruba ayrıldı. Grup l 'deki ratlarda batın açıldı ve abdominal aortları eksplore edildikten sonra hiçbir girişim yapılmadan tekrar kapatıldı (n = 5). Grup 2'de infrarenal aortaya klemp konuldu. 3 saatlik iskemi süresinin sonunda klemp kaldırıldı ve reperfüzyon için 1 saat beklendi (n = 8). Bu işlemin aynısı askorbik asid verilen 3. grupta da uygulandı (n = 8). Bu gruptaki ratlara, işlemden önceki 5 gün boyunca İM olarak 50 mg/kg/gün sodyum askorbat (Redoksan) verildi. Tüm gruplardaki ratlar dördüncü saatte sakrifiye edilerek akciğerleri alındı ve histopatolojik olarak incelendi. Akciğerlerdeki patolojik değişikliklerin derecesini ifade edebilmek için, 0 ile +3 arasında değişen puanlama sistemi kullanılarak hasar skoru hesaplandı.. Bulgular: İkinci ve üçüncü grupta, kontrol grubuna göre daha fazla polimorfonükleer lökosit (PMNL) birikimi, ödem ve konjesyon saptandı. İkinci grubun hasar skor puanı 2.75 olarak bulundu. 3'ncü grubun hasar skor puanı 2.12 olarak hesaplandı. Bu iki grubun sonuçları arasındaki fark istatistiksel olarak anlamlı bulundu (p < 0.05). Sonuç: Askorbik asit, alt ekstremitelerdeki geçici İ/R hasarına bağlı olarak akciğerlerde oluşan patolojik değişiklikleri önlemede yararlı olmaktadır. Background: In this study, injury in the lungs due to experimental lower extremity IR and preventive effect of ascorbic acid were investigated. Methods: Rats were divided into three groups as follows: control group (Group 1), IR group without treatment (Group 2), IR with the administration of ascorbic acid (Group 3). Rats in the group 1 were undertaken laparotomy and closed after exploration of abdominal aorta (n = 5). In the second group, the infrarenal aorta of the rats were clamped a period of 3 hours. After ischémie period, aortic clamping were removed and waited for 1 hour of reperfusion period (n = 8). The rats in the third group were administered 50 mg/kg/day of sodium ascorbic acid commencing from 5 days before experimental procedure (n = 8). The animals were sacrificed and the histopathological specimens of the lungs were examined. The pathological changes in the lungs were ranged between 0+ and 3+ correlated to the level of increasing injury. Results: Edema, congestion and the more prominent accumulation of polymorphonuclear leukocyte (PMNL) were determined in the specimens of group 2 and 3 as compared to the control group. The average score was 2.75 in the group 2. The average score was 2.12 in the third group and it was statistically significant as compared to the second group (p < 0.05). Conclusions: Pretreatment with ascorbic acid was beneficial in prevention of lung injury after IR of lower extremities.
