Akut apandisit tanısında inflamatuvar belirteçlerin yeri
Abstract
Amaç: Bu çalışmanın amacı, akut apandisit ön tanısı ile ameliyat edilen ve 5 farklı histopatolojik gruba ayrılan hastaların, ameliyat öncesi alınan beyaz küre (BK) sayımlarını, C-reaktif protein (CRP) konsantrasyonlarım, nötrofil/granülosit oranlarını (NGO) ve eritrosit sedimantasyon hızlarım (ESH) analiz etmektir. Yöntem: Son iki yıl içerisinde akut apandisit şüphesi ile ameliyat edi.len 110 hasta ileriye dönük olarak değerlendirildi. Hastalardan operasyon öncesi, BK, CRP, NGO ve ESH düzeyleri belirlemek üzere kan örneği alındı. Apandiks histopatolojisi; normal, grade1, 2, 3 ve 4 apandisit olarak sınıflandırıldı. Bu inflamatuvar belirteçler ile histopatolojik bulgular karşılaştırıldı. Bulgular: Ameliyat edilen 110 hastadan 10'unda (% 9) normal histopatolojik bulgular saptanırken, 13'ünde (% 11.8) grade l (minör inflamasyon) apandisit, 29'unda (% 26.4) grade 2 (inflame) apandisit, 47'sinde (% 42,8) grade 3 (gangrene) apandisit, l Tinde (% 10) grade 4 (perfore) apandisit saptandı. B K ilerleyen apandiks inflamasyonunun belirteci olmadı. CRP'nin apandiks inflamasyonundan sonra belirgin olarak arttığı ve histopatolojik bulguların ilerlemesi ile birlikte artmaya devam ettiği görüldü. NGO ve ESH'nin apandiks inflamasyonu sonrasında belirgin olarak arttığı, fakat histopatolojik bulguların ilerlemesi ile birlikte artış göstermediği saptandı. Sonuç: CRP düzeyi apandisit inflamasyonunun erken bir belirtecidir, üstelik tekrarlanan CRP ölçümleri akut apandisit tamamdaki doğruluk oranım artırabilir. Aim: To analyze the preoperative white blood cell count (WBC), C-reactive protein (CRP) concentration, fraction of neutrophil granulocytes (neutrophil count), and erythrocyte sedimentation rate (ESR), of the patients who were operated on for a clinical suspicion of acute appendicitis and who were separated into five groups with different findings at histopathologic examination. Method: 110 patients, who were operated on for suspected acute appendicitis in the last two years were evaluated prospectively. Blood was collected preoperatively from the patients for the measurement of serum CRP, WBC, ESR and neutrophil count. The histopathology of appendices were grouped into normal, grade 1,2,3 and 4 appendicitis. The relationship between inflammatory markers and histopathologic findings were examined. Results: In 10 (9 %) of the 110 patients operated on, normal histopathologic findings were confirmed, in 13 (11.8 %) appendix was grade 1 (minor inflammation), in 29 (26.4 %) appendix was grade 2 (inflammed), in 47 (42.8 %) appendix was grade 3 gangrenous and in 11 (10 %) appendix was grade 4 (perforated). WBC was not a marker of increasing appendiceal inflammation. CRP increased markedly after appendiceal inflammation and increased gradually with histopathologic findings. Neutrophil count and ESR increased significantly after appendiceal inflammation but did not increase parallel to histopathologic findings. Conclusion: CRP level is an early marker of appendiceal inflammation and moreover repeated controls of the CRP concentrations may improve the accuracy of the diagnosing acute appendicitis.
