Hipotiroidi ile seyreden bir konjenital guatr olgusu: Tiroid sintigrafisi ve Tc-99m tutulumunun önemi

Abstract

Bu makalede boyunda belirgin solunum zorluğuna neden olan kitle ile doğan konjenital guatrlı bir yenidoğan bebek sunulmaktadır. Boyundaki kitlenin hiperplazik tiroid dokusu olup olmadığının araştırılması amacıyla Nükleer Tıp Anabilim Dalı'na gönderilen olguda; Teknesyum-99m (Tc-99m) perteknetat kullanılarak yapılan tiroid sintigrafisinde bütün boyun bölgesini kaplayacak şekilde belirgin hiperplazik bir tiroid bezi olduğu ve sintigrafik yöntemle yapılan tutulum hesabı sonucunda, artmış Tc-99m perteknetat tutulumu (% 28.2) gösterdiği saptandı. Sonuç olarak, Tc-99m perteknetat ile yapılan tiroid Sintigrafisi ve tiroid bezinde sintigrafik olarak tutulumun hesaplanması, konjenital hipotiroidinin sebebinin ortaya konmasında ve tiroid bezi hiperplazisi şüphelenilen neonatal olgularda doğruluğu kanıtlanmış, kolay uygulanabilir ve faydalı bir yöntem olarak gözardı edilmemelidir.

We report here a case of congenital goitre in a newborn baby with mass in the neck which causes significant respiratory distress. The patient was referred to Dept. of Nuclear Medicine to investigate vvhether this mass was hyperplastic thyroid tissue. Thyroid gland was significantly hyperplastic and surrounding almost the entire neck, according to the result of thyroid scintigraphy by Technetium-99m (Tc-99m) pertechnetate. it was also found that Tc-99m pertechnetate uptake increased (28.2%). in conclusion, thyroid scintigraphy with Tc-99m pertechnetate and measurement of Tc-99m uptake are accurate, easily applicable and useful technigues in diagnosis of congenital hypothyroidism and in clinical evaluation of hyperplastic thyroid tissue in newborns.
