dc.description.abstractCumhuriyet üniversitesi hastanesine 1998 ile 2001 yılları arasındaki dört yıllık süre içinde yattığı sırada nozokomiyal üriner sistem infeksiyonu (NÜSİ) gelişen olguların idrar kültürlerinden izole edilen etken mikroorganizmalar ile bu mikroorganizmalara ait antimikrobiyal direnç durumları ileriye dönük olarak araştırıldı. Mikroorganizmalar Sceptor (Becton Dickinson Diagnostic Instrument Systems, Tovvson, USA) otomatik sistem ile tanımlanarak yine aynı sistemle antimikrobiyal direnç durumları saptandı. Takip süresi içinde 353 hastada oluşan 380 NÜSİ atağında idrar kültürlerinden 316 etken mikroorganizma izole edildi. . En sık izole edilen mikroorganizmalar sırasıyla Escherichia coli (%37.7), Klebsiella spp (%17.7), Candida spp (%12.7) ve Pseudomonas aeruginosa (%6.9) idi. E coli' lerin sıklıkla dirençli olduğu antibiyotikler sırasıyla piperasilin (%63), tetrasiklin (%59) mezlosilin (%57) ve ampisilin (%50) iken bu mikroorganizmalarda meropenern ve sefepim direnci gözlenmedi. İkinci sıklıktaki NÜSİ etkeni olan Klebsiella suşlarının dirençli olduğu antibiyotikler sırasıyla ampisilin (%86), amoksisilin-klavulanat (%80) ve mezlosilin (%68) idi. Bu suşların da sefepime direnç göstermediği ve sıklıkla meropenem (%95) ve imipeneme (%93) duyarlı oldukları belirlendi. P. aeruginosa suşlarında sıklıkla sefotaksim (%77) ve gentamisin (%59) direnci olduğu tespit edildi. Antipsödomonal üçüncü kuşak bir sefalosporin olan seftazidim direnci %36 olarak bulundu. Bu suşlar da sıklıkla imipenem (%91) meropenem (%91), sefepim (%86) ve siprofloksasine (%82) duyarlı idi.en_US
dc.description.abstractCausative microorganisms and its antimicrobial resistance patterns isolated from the patients who acguired nosocomial urinary tract infections (NUTIs) during the hospitalization, a four year period between January 1998 and December 2001, in Cumhuriyet University Hospital were examined prospectively. Microorganisms were identified and antimicrobial resistance patterns were ascertained by Sceptor (Becton Dickinson Diagnostic Instrument Systems, Tovvson, USA) automatic system. During the study period 316 microorganisms were isolated from the urine cultures at 380 NUTI attacks in 353 patients. The predominant microorganisms were Escherichia coli (37.7%), Klebsiella spp (17.7%), Candida spp (12.7%), and Pseudomonas aeruginosa (6.9%) respectively. High resistance rates against piperacillin (63%), tetracycline (59%), mezlocillin (57%), and ampicillin (50%), but no resistance to meropenem and cefepime were observed in E.Coli strains. Klebsiella spp, the second frequent NUTI causing microorganisms, showed resistance to ampicillin (86%), amoxicillin-clavulanate (80%), and meziocillin (68%). These isolates had no resistance to cefepime, and susceptible to meropenem (95%) and imipenem (93%). Freguent resistance rates were found against cefotaxime (77%) and gentamicin (59%) in the P. aeruginosa strains. Ceftazidime, a third generation antipseudomonal cephalosporin, resistance was 36% in these microorganisms which were susceptible to imipenem (91%), meropenem (91%), cefepime (86%), and ciprofloxacin (82%) respectively.en_US
