Kronik böbrek yetmezliği olan olgularda gelişen adrenal yetmezliğin tanısal güçlüğü: Olgu sunumu

Abstract

Kronik böbrek yetmezliği olanlarda sonuçlar çelişkili olsa da çoğunlukla plazma kortizol düzeyleri artmış olarak bulunmuştur. Akciğer, lenf bezi, böbrek tüberkülozu ve buna bağlı gelişen kronik böbrek yetmezliği ile takip edilen bir hastada, adrenal yetmezlik kliniği olmasına karşın bazal ve ACTH uyarı testi ile ölçülen plazma kortizol düzeyleri normal bulundu. Ancak hasta adrenal yetmezlik tedavisine cevap verdi. Enzim veya radio immünoessey yöntemi kullanıldığında kronik böbrek yetmezliğinde birikimi söz konusu olan serbest ve konjüge kortizol deriveleri kortizol antikorları ile çapraz tepki verebildiğinden, muhtemelen plazma kortizolü olduğundan daha yüksek ölçülmektedir. Bu nedenle kronik böbrek yetmezliği olan ve klinik olarak kuvvetle adrenal yetmezlik düşünülen KBY olgularında ölçülen kortizolün, gerçek kortizol düzeylerini yansıtmayabileceği hatırda tutulmalıdır.

Basal plasma cortisol levels in patients with chronic renal failure mostly have been found to be elevated although there are discrepancies results. in a patient follovved up with pulmonary, lymph node and renal tuberculosis causing chronic renal failure, plasma cortisol levels in the basal and response to ACTH stimulation test were found to be normal although there was the clinic of adrenal insufficiency. However, the patient responded to the treatment for adrenal insufficiency. Because of free and conjugated cortisol derivates could accumulate in chronic renal failure and they could be expected to cross react with cortisol antibodies used in the enzyme and radio immunoassay procedures, measurement of plasma cortisol levels probably may be overestimated. For this reason, it should be considered that measurement of cortisol levels might not reflect true cortisol levels in the patients with chronic renal failure who have strongly suspected adrenal insufficiency.
