Klinik örneklerden soyutlanan staphylococcus aureus ve koagülaz negatif stafilokok izolatlarında mupirosin direnci
Abstract
Mupirosin, Gram pozitif bakteriler ve özellikle Staphylococcus aureus’un neden olduğu yüzeyel infeksiyonların tedavisinde topikal antimikrobiyal ajan olarak kullanılmaktadır. Bu çalışmada, klinik örneklerden soyutlanan 183 S.aureus ve 150 koagülaz negatif stafilokok (KNS) kökeninin mupirosine direnci minimum inhibitör konsantrasyon (MİK) belirlenmesi ile araştırılmıştır. S.aureus kökenlerinin 98’i ( % 53.6) metisiline dirençli, 85’i (% 46.4) metisiline duyarlı ve KNS kökenlerinin ise 66’sı (% 44) metisiline dirençli, 84’ü (% 56) metisiline duyarlı bulunmuştur. Düşük düzey mupirosin direnci, metisiline dirençli ve duyarlı S.aureus (MRSA ve MSSA) kökenlerinde sırası ile % 31.6 ve % 10.6, metisiline dirençli ve duyarlı KNS kökenlerinde ise sırası ile % 12.1 ve % 2.4 olarak saptanmıştır. Yüksek düzey mupirosin direnci, metisiline dirençli ve duyarlı KNS kökenlerinde sırası ile % 4.5 ve % 1.2 olarak bulunmuş olup, S.aureus kökenlerinde saptanmamıştır. Çalışmamızda elde edilen bulgular, MRSA kökenlerinde mupirosine karşı dirençte bir artış olduğunu düşündürmektedir. Mupirocin is used as a topical antimicrobial agent in the treatment of superficial infections caused by Gram-positive bacteria, particularly by Staphylococcus aureus. The aim of this study is to determine the in vitro efficacy of mupirocin against staphylococci isolated from clinical specimens. Mupirocin resistance of 183 S.aureus and 150 coagulase-negative staphylococci (CNS) was investigated by the determination of minimum inhibitory concentration (MIC). Ninety-eight (53.6 %) of S.aureus strains were found methicillin resistant, 85 (46.4 %) methicillin susceptible. Sixty-six (44 %) of CNS strains were found methicillin resistant, 84 (56 %) methicillin susceptible. Low level mupirocin resistance was detected as 31.6 % and 10.6 % in methicillin resistant and susceptible S.aureus (MRSA and MSSA) strains, as 12.1 % and 2.4 % in methicillin resistant and susceptible CNS strains, respectively. High level mupirocin resistance was observed in 4.5 % and 1.2 % in methicillin resistant and susceptible CNS, respectively, whereas not detected in any of S.aureus strains tested. These results implies an increase in mupirocin resistance in MRSA strains.
