İkinci trimester gebelik sonlandırılmasında farklı misoprostol uygulamalarının abortus süresi ve kullanılan ilaç dozu açısından etkinliğinin karşılaştırılması

Abstract

Amaç. Bu çalışmanın amacı ikinci trimester gebelik sonlandırmasında altı saatte bir ve dört saatte bir oral ve vajinal yolla uygulanan sırasıyla 400 ?g ve 200 ?g misoprostolün etkinliğini karşılaştırmak. Yöntem. Geriye doğru dosya incelemesi şeklinde planlanan çalışmamızda kliniğimizde uygulanmakta olan misoprostol tedavi protokollerinin etkinliği incelenmiştir. 18-43 yaş arası, ikinci trimester gebeliğin sonlandırılmasının planlandığı toplam altmış kadının dosyası değerlendirilmiştir. 33 hastaya (grup 1) 6 saat ara ile tekrarlanan 200 µg oral ve 200 µg vajinal yolla misoprostol uygulanmıştır. 27 hastaya (grup 2) 4 saat ara ile tekrarlanan 100 µg oral ve 100 µg vajinal yolla misoprostol uygulanmıştır. Dışlanma kriterleri sistemik hastalık, servikal operasyon öyküsü, mevcut gebelik sırasında kanama veya lekelenme idi. Her iki grupta yeterli uterin kontraksiyon elde edilene kadar doz tekrarlanmıştır. Kırk sekiz saatte düşük oluşmadı ise yöntem başarısız kabul edilmiştir. Bulgular. Grup 2’nin ortalama indüksiyon düşük aralığının grup 1’inkinden anlamlı düzeyde kısa olduğu bulunmuştur (16,6±8,8 saate karşın 10,3 ±7,8 saat) (p<0,05). Grup 2’de kullanılan ortalama toplam misoprostol dozunun grup 1’inkinden anlamlı düzeyde daha düşük olduğu saptanmıştır (p<0,05). Gruplarda hiçbir yan etki tespit edilmemiştir. Sonuç. İkinci trimester gebelik sonlandırılmasında sık aralıklarla uygulanan düşük doz misoprostolün, uzun aralıklarla uygulanan yüksek dozdan daha etkili olduğu bulunmuştur.

Aim. To compare the effectiveness of 400 &amp;#956;g and 200 &amp;#956;g misoprostol administered by vaginal and oral route, repeated every six hours and four hours, respectively for second trimester pregnancy termination. Methods. In this study, which is a retrospective file review, we evaluated the efficacy of misoprostol treatment protocols used in our clinic. Medical files of a total of sixty women, aged 18-43 years, who was planned to have terminated second trimester pregnancy, were evaluated. Thirty three patients (group 1) received 200 &micro;g oral and 200 &micro;g vaginal misoprostol, repeated every 6 hours. Twenty seven patients (group 2) received 100 &micro;g oral and 100 &micro;g vaginal misoprostol, repeated every 4 hours. Exclusion criteria were systemic disease, a history of cervical operations, bleeding or spotting during the current pregnancy. If abortion did not occur within forty eight hours, the treatment was considered to be failed. Results. The mean induction&amp;#8211;abortion interval of the group 2 was found to be significantly shorter than of the group 1 (16.6&plusmn;8 h versus 10.3 &plusmn;7.9 h). The mean total dose of misoprostol of group 2 was significantly lower than of the group 1 (group 1, 1200&plusmn;529&micro;g; group 2, 622&plusmn;390 &micro;g). There were no side effects in the groups. Conclusion. It was found that lower dose of misoprostol repeated in short intervals was more effective than of the higher doses repeated in long intervals.
