Amaç: Diyabetik ve diyabetik olmayan katarakt hastalarında hümör aközde ve eş zamanlı venöz kan serumunda, total oksidatif stres (TOS), total antioksidan kapasite (TAK), paraoksonaz (PON1), arilesteraz (ARE), lipid peroksidaz (LPO) seviyelerini karşılaştırmak ve diyabetik retinopatinin ilerlemesinde oksidatif stresin rolünü değerlendirmek. Gereç ve Yöntem: Oksidatif doku ve organ hasarı, diyabette ve komplikasyonlarında rol oynar. Hem kanda hem de lens örneklerinde süperoksit dismutaz, glutatyon peroksidaz, glutatyon redüktaz ve katalaz gibi antioksidan enzim aktivitelerindeki azalmalar bildirilmiştir. PON1, ARE, LPO şimdiye kadar incelenmeyen antioksidan enzimlerdir. Kliniğimize puslu görme şikayeti ile başvuran ve katarakt tanısı alıp, cerrahi tedavi önerilen diyabetik ve diyabetik olmayan hastalar dahil edildi. Diyabeti olup retinopatisi olmayan hastalar 1. grup, diyabetik retinopatili hastalar 2. grup ve katarakt tanısı dışında ek sistemik hastalığı olmayan hastalar 3. grup olarak ayrıldı. Her bir grup 26 hasta olacak şekilde toplam 78 hasta çalışmaya dahil edildi. Sonuçlar: Bu çalışmada hümör aköz TOS, TAK, PON1 ve ARE ve LPO düzeyleri açısından gruplar arasında istatistiksel olarak anlamlı bir fark bulunmadı. Her 3 grubun eş zamanlı venöz kan serumu sonuçları, TOS, TAK, PON1 ve HbA1c düzeyleri istatistiksel olarak önemli bulunmuşken, ARE ve LPO açısından önemsiz bulundu. Grup 1 ve 2, kontrol grubu ile karşılaştırıldığında, Grup 1’de istatistiksel olarak anlamlı oranda PON 1 düşük, HbA1c yüksek, Grup 2’de TOS ve HbA1c yüksek, TAK ve PON 1 düşük bulundu (P<0,05). Tartışma: Oksidatif stres göstergesi enzimler kanda pozitif, hümör aközde ise negatif olarak tespit edildi. Hümör aközde ilk kez çalışılan bu enzimlerde, polimorfizm gösterenlerin alt tiplerinin de dahil edildiği yeni çalışmalara ihtiyaç olduğu düşünüldü. Pur po se: The aim was to compare total oxidative stress (TOS), total antioxidant capacity (TAC), and paraoxonase (PON1), arylesterase (ARE), and lipid peroxidase (LPO) levels in the aqueous humor and concurrent venous blood serum and to evaluate the role of oxidative stress in the progression of diabetic retinopathy in diabetic and non-diabetic patients with cataract. Ma te ri al and Met hod: Oxidative tissue and organ damage play a significant role in diabetes and related complications. A reduction in the activities of antioxidant enzymes such as superoxide dismutase, glutathione peroxidase, glutathione reductase and catalase has been reported in both blood and lens samples. PON1, ARE, and LPO are unreviewed antioxidant enzymes until now. We included diabetic and non-diabetic patients who were admitted to our clinic for complaints of hazy vision and who received the diagnosis of cataract. Patients were divided into 3 groups: group 1 - diabetic patients without retinopathy, group 2- with diabetic retinopathy, and group 3 - patients without systemic disease outside the diagnosis of cataract. A total of 78 patients (26 patients in each group) were enrolled in the study. Re sults: In this study, there was no statistically significant difference among the groups for TOS, TAC, PON1, ARE and LPO levels in aqueous humor. Although simultaneous venous blood serum results, TOS, TAC, PON1 and HbA1c levels in all 3 groups were found statistically significant, ARE and LPO were not statistically significant. When groups 1 and 2 were compared with control group, it was found that in group 1, PON 1 was statistically significantly low and HbA1c was high, while in group 2, TOS and HbA1c were high and TAC and PON 1 were low (p<0.05). Dis cus si on: Oxidative stress indicator enzymes were determined to be positive in blood and negative in humor aquosus. In the present study, these enzymes were studied for the first time in aqueous humor, thus, further studies including subtypes in case of polymorphism are needed.
