Konjenital Toksoplazmozisli Bir Olgu Sunumu

Abstract

Hamilelik esnasında annede oluşan primer toksoplasma gondii enfeksiyonu transplasental yolla fetusa geçer ve konjenital enfeksiyona ve sekellere neden olur. Bu çalışmada sunduğumuz 48 günlük bebek santral sinir sistemi enfeksiyonu ön tanısıyla pediatri servisinde hospitalize edilmiş ve intrauterin enfeksiyon ve koryoretinit şüphesiyle kliniğimize konsülte edilmiştir. Hastanın yapılan muayenesinde her iki gözde ışık ve obje takibinin mevcut olduğu ve ön segment muayene bulgularının normal olduğu görüldü. Fundus muayenesinde her iki gözde makülada yaklaşık 1.5 optik disk çapı büyüklüğünde etrafı skarlaşmış ortası aktif retinit odağı görüldü. Tomografide; posterior fossa yapıları sığ, periventriküler alanlarda,bazal ganglionlarda yaygın kalsifikasyon odakları ve ventriküler sistem dilate olarak tespit edildi. Toksaplazma Ig G ve M pozitif olarak bulundu. Hastaya spiramisin 2x175 mg, trimetoprim+sulfometaksazol 2x11mg ve antibiyotiklerden 3 gün sonra dekort amp 4x0.45 mg tedavi olarak başlanıldı. Ancak 2 hafta sonra hastanın genel durumu bozuldu, ışık ve obje takibini yapamadığı görüldü ve daha sonra birkaç gün içerisinde kaybedildi. Sonuç olarak konjenital toksoplazmozisli olguların çoğu asemptomatik olsada ciddi sekellerden kaçınabilmek ve erken tanı ve tedavi için özellikle ilk tremesteri kapsayacak şekilde rutin gebe takiplerinin yapılması gerektiğini düşünmekteyiz.

Primary Toxoplasma Gondii infection of the mother during pregnancy can cause congenital infection and sequela in the fetus via transplacental passage. In this case report we would like to present a 48 days old infant who was hospitalised with initial diagnosis of central nervous system infection and was consulted to ophtalmology department with suspicion of chorioretinitis and congenital infection. In ophtalmologic examination, the light and object pursuit in both eyes were present and the anterior segment was normal. Fundus examination revealed retinitis focus with scaring edges and active center in both macula with a diameter of 1.5 times the size of the optic disc. At computerised tomography of the brain shallow posterior fossa, diffuse calcification foci in basal ganglia and periventricular regions and dilatation in ventricular system was noticed. Toxoplasma IgG and IgM was positive. Spiramycin 2x175 mg, trimetoprim+sulfometaksazol 2x11 mg was given as antibiotheraphy and 3 days after the antibiotics dexametasone 4x0.45 mg was added. Two weeks later the patient worsened and could not perform the light and object pursuit and died in a few days time. In conclusion we think that even though most of the cases with congenital toxoplasmosis are asymtomatic, routine examination of the pregnant woman including the first trimester are necessary to prevent the catastrophic sequela via early diagnosis and treatment.
