Kapsüler glokomda trabekülektomi sonuçları

Abstract

AMAÇ: Bu çalışma kapsüler glokom (KG) lu hastalarda trabekülektominin göz içi basıncı (GİB)'na etkisini araştırmak için yapıldı. YÖNTEM: 1990-1995 yılları arasında trabekülektomi uygulanan, 44'ü (52 göz) kapsüler, 44'ü (58 göz) primer açık açılı glokom (PAAG) lu 88 (110 göz) hasta çalışma kapsamına alındı. İki grubun preoperatif ve postoperatif ortalama GİB değerleri ve GİB kontrolü sağlaması anlamına gelen postoperatif GİB'nın 16mmHg ve altına düşmesi açısından iki grup incelendi. BULGULAR: Preoperatif ortalama GİB KG grubunda PAAG'lu hastalara göre daha yüksek olmasına karşın (sırasıyla 36.98±9.07mmHg, 33.48±8.53mmHg, p<0.05), postoperatif ortalama GİB KG'lu hastalarda daha düşüktü (sırasıyla 14.71±7.38mmHg, 15.44±5.07mmHg, p>0.05). KG'lu hastaların postoperatif G.ayda %87.2'sinde, S.yılda %68.4'ünde GİB'ı 16mmHg ve altında idi. Bu oranlar PAAG'lu hastalarda sırasıyla %85.2 ve %64.5 idi. İki grup arasında postoperatif dönemde, GİB kontrolü açısından anlamlı bir fark saptanmadı (P>0.05). i SONUÇ: Bu sonuçlar ile, trabekülektominin kapsüler glokomlu hastaların GIB'nı düşürmede PAAG'lu hastalardaki kadar başarılı olduğu düşünüldü.

PURPOSE: This study is performed to evaluate the effect of trabeculectomy on intraocular pressure (IOP) in patients with capsular glaucoma (CG). METHODS: In the period 1990-1995 44 capsular glaucoma (58 eyes) and 44 primary open-angle glaucoma (POAG)(52 eyes), totally 88 patients (110 eyes), performed trabeculectomy, were included in this research. In both groups the mean values of preoperative and postoperative IOP were compared and also the adequate control of IOP, means decreasing the IOP under the level of 16mmHg postoperatively, were evaluated. RESULTS: The mean values of IOP in CG patients were higher than those of POAG patients preoperatively (36.98&plusmn;9.07 mmHg and 33.48&plusmn;8.53mmHg respectively, p&lt;0.05) although the mean values of IOP in CG patients were lower than those of POAG patients postoperatively (14.71&plusmn;7.38mmHg and 15.44&plusmn;5.07 mmHg respectively, p&gt;0.05). In CG patients, IOP values in 87.2% of cases on 6th month and 68.4% of cases on 5th years were below the 16 mmHg. And in POAG patients, these percentages were 85.2% and 64.5% respectively. There was no significant differences in control of IOP between two groups during follow-up period. CONCLUSION: As a result it is concluded that trabeculectomy is a satisfactory method in decreasing the IOP of patients with capsular glaucoma as well as primary open-angle glaucoma.
