Romatoid artritte solunum sistemi tutulumu

Abstract

Bu çalışma romatoid artrit (RA)'te solunum sistemi tutulumunun sıklığını, tipini ve saptanan bulguların hastalığın diğer özellikleri ile ilişkisini araştırmak amacıyla düzenlenmiştir. Ortalama yaşları 54.5 (27-70), ortalama hastalık süreleri 8.5 (0.08-25) yıl olan 25 RA'lı olguda (16 kadın, 9 erkek) hastalık öyküsü alınıp ayrıntılı sistem sorgulaması ve fizik muayene ile birlikte toraks grafisi, ince kesit torakal bilgisayarlı tomografi (BT), solunum fonksiyon testi (SFT), gereken hastalarda plevral sıvı incelemesi ve bronkoskopi yapıldı. Beş hastada d iff üz interstisyel fibrozis (% 20), 4 hastada (% 16) plevral tutulum, bir hastada (% 4) intrapulmoner nodul ve bir hastada (% 4) metotreksat (MTX) pnömonitisi saptandı. Plevral tutulumlu hastaların tümü erkek, parankimal akciğer hastalığı gelişen 7 olgudan 6'sı kadındı. Olguların tümünde periferik artrit vardı. Sadece bir hastada subkutan nodul gözlendi. Solunum sistemi tutulumu saptanan 11 hastanın 10'unda serum romatoid faktör (RF) pozitif idi. Akciğer tutulumu ile RF pozitifliği ve cinsiyet arasında anlamlı ilişki bulunmadı. Solunum sistemi tutulumunun RA süresi 5 yılın altında olanlarda 5 yılın üstünde olanlara göre daha sık ortaya çıktığı görüldü.

PULMONARY INVOLVEMENT IN RHEUMATOID ARTHRITIS This study was performed to evaluate the prevalence and the type of pulmonary involvement in rheumatoid arthritis (RA) and to search the relationship between the defined symptoms and other features of the disease. Disease history was acquired, and a detailed rewiew of the systems, and a thgrough physical examination were done in 25 patients (16 women, 9 men) with RA, whose mean age was 54.5 (27-70) and mean disease duration was 8.5 (0.08-25) years. Thorax X-ray, high resolution computed tomography of the thorax and pulmonary function tests were performed in all of the patieqts and bronchoscopy and pleural fluid analysis were done in required cases. Diffuse interstitial fibrosis, pleural disease, intrapulmonary nodule and methotrexate (MTX) pneumonitis were determined in 5 (20 %), 4.(16 %), 1 (4 %), and 1 (4 %) out of 25 patients, respectively. All patients with pleural disease were male, wheras 6 out of 7 patients with parenchymal pulmonary disease were female. All cases had peripheral arthritis. Subcutaneous nodule was found in only 1 patient. Serum rheumatoid factor (RF) was positive in 10 out of 11 patients with pulmonary involvement. Pulmonary involvement was found to have no significant correlation with either RF positivity or sex. Pulmonary inovolement was observed to be more common in patients with disease duration of less than 5 years than those with that of more than 5 years.
