Ezotropya tedavisinde geniş unilateral medial rektus geriletmesi

Abstract

Cumhuriyet Üniversitesi Tıp Fakültesi Göz Hastalıkları Anabillm Dalı'nda 30 ile 35 prizm dioptri (PD) arasında ezotropyası olan 17 olguya unilateral olarak medial rektus adalesine 6.5 mm geriletme uygulandı. Olguların çoğunda (%70.5) ambliyopi mevcuttu. Olgular ortalama 6.5 ay (3.5-16) takip edilerek son kontrollerinde 12 PD'den küçük ezotropyası olanlar başarılı kabul edildiler. Postope-ratif rezidüe ezotropya ortalama 7.1 PD (0-20) olarak saptandı. Or¬talama 23.8 PD (15-32)'lik düzeltme sağlayan bu cerrahi girişim ile 17 olgunun 15 (%88)'inde yeterli kabul edilen sonuçlara' '-Jaşıldı. Hiçbir olguda addüksiyonda kısıtlılık ve konverjansda bozulma gö¬rülmedi. Bu sonuçlar özellikle sekonder (konsekütif) ezotropyaya meyilli ambliyopik ezotroplarda standart cerrahi girişim olan geri-letme-rezeksiyona göre unilateral geniş medial rektus geriletmesi¬nin yeterli ve daha avantajh olduğunu düşündürmektedir.

At the Department of Ophthalmology in Cumhuriyet University Faculty of Medicine 17 esotropic patients with deviation of 30 to 35 prism diopters (PD) were treated with unilateral 6.5 mm medial rectus reces¬sion. In this study 70.5% of the patients had amblio-pia due to the impaired vision on the strabismic eye. Fifteen of the 17 patients (88%) were straight posto-peratively 6.5 months (3.5-16 months) follow up pe¬riod or had 12 PD or less of esotropia. The mean pos¬toperative deviation was 7.1 PD (0-20 PD) and the average amount of correction was 23.8 PD. No com¬plications such as adduction deficiency and conver¬gence insufficiency were observed. In the treatment of moderate angle esotropia, especially in the amblio-pic eyes that tends to seconder exotropia, this proce¬dure seems to be more effective and safe than stan- -dart recession-resection technique.
