Laboratuvarımızda hazırladığımız elisa kiti ile akciğer tüberkülozunun serolojik tanısı
Abstract
Son yıllarda gittikçe önem kazanan, güvenilir, basit, hızlı ve tekrarlanabilir tanı yöntemlerinden olan ELlSA tekniğinin tüberkülozun serolojik tanısında ne oranda etkili olabileceğini kendi laboratuvar koşullarımızda hazırladığımız kit ile, çeşitli gruplar üzerinde deneyerek saptamayı amaçladık. Çalışmada deney grubu olarak akciğer tüberkülozu tanısı konmuş aktif ve kronik 99 hastadan elde edilen serum örnekleri kullanıldı. Kontrol olarak PPD (-) ve PPD (+) olan 30'ar kişilik iki grup deneye alınmıştır. Hasta ve kontrol gruplarının IgG düzeylerinin ortalama absorban değerleri arasında anlamlı fark tespit edildi. MSAA antijeni ve BSA blok çözeltisi kullanıldığında ELlSA testinin duyarlılığı %96.0, seçiciliği ise %93.3 olarak sap-tandı. Yalancı pozitiflik oranı %6.7, yalancı negatiflik oranı ise %4.0 olarak bulundu. MSAA antijeni ve Jelatin blok çözeltisi kullanıldığında ELlSA testinin duyarlılığı %87.0 ve seçiciliği %81.0 olarak tespit edildi. Yalancı pozitiflik oranı %13.0, yalancı negatiflik oranı ise %19.0 olduğu görüldü. Oranlar arasındaki farkın istatistiksel olarak anlamlı ölçüde yüksek olduğu sap-tandı (p<0.01). Sonuç olarak ELISA tekniğinin tüberkülozun serolojik tanısında ve tarama çalışmalarında tercih edilebile-cek bir serolojik tanı yöntemi olduğu düşünüldü. In the recent years, ELISA has become an important technique since it is a rapid, simple, reliable and reproducible method. The aim of the present study was to detect the efficiency of ELISA in the diagnosis of tuberculosis and using kit which was produced in our laboratory. We used the sera of 99 pulmoner tuberculosis patients. From these patients 30 had no therapy yet, 31 were taken under therapy for two months and the rest were the chronic pulmoner tuberculosis cases. We had two control group as PPD(+) and PPD(-) with 30 volunteer participants in each group. There was a significant difference between mean absorbance values of IgG levels of the patient and control groups. The sensitivity and specificity of ELISA test using BSA block solution were found as 96.0% and 93.3% respectively. The rate of false (+) was 6.7% and the rate of false (-) was 4.0%. The sensitivity and specificity of ELISA test using Jelatin block solution were found as 87.0% and 81.0% respectively. The rate of false (+) was 13.0% and the rate of false (-) was 19.0%. The difference between these rates were sta-tistically significant (p<0.01) In our opinion ELISA can be a method of choice in the diagnosis of tuberculosis because of its rate of sensitivity and specificity, rapidity, reproducibility and simplicity of use.
