dc.description.abstractTiroid fonksiyon bozukluklarının depresyon, anksiyete gibi psikiyatrik rahatsızlıkları artırdığı, yaşam kalitesinde ise azalmalara sebep olduğu bilinmektedir. Standart ölçeklerle bunları değerlendirmek mümkündür. Beck Anksiyete, Beck Depresyon ve SF-36 ölçekleri yardımıyla Sivas Cumhuriyet Üniversitesi Endokrinoloji bölümüne başvuran 20-80 yaş arası yaşamının bir döneminde hipotiroidi tanısı almış şuan L-tiroksin tedavisi ile ötiroid halde olan 40 Hashimoto tiroiditi tanılı, 40 herhangi bir nedenle tiroid cerrahisi sonrası hipotiroidi gelişen, 40 sağlıklı kontrol grubu olmak üzere 120 kişi üzerinde anksiyete, depresyon ve yaşam kalitesi değerlenlendirilmesi yapıldı. Çalışmada kişilerin bu psikososyal etkilenmelerindeki nedenler tiroid fonksiyon bozukluğunun yanı sıra; kronik hastalığa sahip olma düşüncesi, sürekli ilaç kullanımı düşüncesi, otoimmünitenin etkisi ve endojen hormon salgısının etkisi gibi çok yönlü olarak değerlendirildi. Hashimoto tiroiditli ve tiroidektomili kişiler kontrol grubuna göre daha yüksek oranda anksiyete ve depresyona sahipken, yaşam kaliteleri de daha düşük olarak saptandı (p<0,05). Endojen tiroid hormon salgısının etkisi ve/veya tiroid bezinin henüz aydınlatılamamış bez varlığının etkisine bağlı olarak Hashimoto tiroiditli hastaların tiroidektomi olmuş hastalara göre anksiyete ve depresyon oranının ve düzeyinin daha düşük, yaşam standartının genel anlamda daha yüksek olduğu saptandı. Bu nedenle tarafımızca tiroid bezinin ruhsal ve bedensel sağlığa olumlu etkilerinden dolayı bezin korunması önerilmektedir. dc.description.abstractIt is known that disorders of thyroidal function increases psychiatric diseases such as depression, anxiety, and decreases the quality of life. It is possible to determine these by standard scales. Beck Anxiety, Beck Depression and SF-36 computer software scales were applied to a total of 120 subjects aged 20-80 of whom 40 were diagnosed as Hashimoto thyroiditis at any time in their life in the Endocrinology Department of Cumhuriyet University Hospital and have become euthyroid by L-thyroxin treatment after then; 40 subjects who had hypothyroidism due to various thyroid surgeries and finally the last 40 subjects constituted healthy control group. In addition to thyroidal dysfunction, the reasons for these psychosocial effects of the people in the study such as the thought of having a chronic disease, the thought of continuous drug usage, the effect of autoimmunity and the effect of endogenous hormone secretion were also evaluated in a multifaceted way. While patient with Hashimoto thyroiditis and thyroidectomy had higher rates of anxiety and depression than the control group, their quality of life was also found to be lower (p<0,05). Due to the effect of endogenous thyroid hormone secretion and / or the effect of the presence of a gland that has not yet been clarified, the patients with hashimoto thyroiditis were found to have a lower rate and level of anxiety and depression, and a higher standard of living in general compared to patients who underwent thyroidectomy. Therefore, the preservation of thyroid gland is recommended due to its positive effects on mental and physical health.
