Bir üniversite hastanesinde depremzede hastalara hizmet veren sağlık çalışanlarının ikincil travmatik stres, depresyon ve anksiyete düzeylerinin değerlendirilmesi

Özet
Türkiye'de 6 Şubat 2023 tarihinde meydana gelen Kahramanmaraş merkezli depremler, çok sayıda can kaybına ve yaralanmaya neden olmuştur. Deprem bölgesinden sevk edilen ve Sivas iline yerleştirilen depremzedelere hastanemizde ikinci ve üçüncü basamak sağlık hizmeti sunulmuştur. Afet durumlarında görev yapan meslek gruplarında, travma mağdurlarıyla etkileşimleri sonucunda çeşitli ruhsal belirtiler gelişebilmektedir. Bu çalışmada; hastanemizde depremzede hastalara hizmet veren sağlık çalışanlarının ikincil travmatik stres, depresyon, anksiyete düzeylerinin değerlendirilmesi amaçlanmıştır. Kesitsel tipteki bu araştırmaya, depremzede hastalara hizmet veren birimlerde görev yapan sağlık çalışanları arasından kolayda örnekleme yöntemi kullanılarak belirlenen; 228 doktor, 74 intörn doktor, 330 hemşire olmak üzere toplam 632 sağlık çalışanı dahil edildi. Katılımcılarımızın sosyodemografik ve mesleki özellikleri, ikincil travmatik stres ölçeği (İTSÖ) ve depresyon, anksiyete, stres ölçeği-21 (DASÖ-21) ile değerlendirildi. Katılımcılarımızın ikincil travmatik stres ölçeği toplam puan ortalaması 42.5 ± 14.4; DASÖ-21 depresyon puanı ortalaması 6.7 ± 5.7 olup, depresyon ölçek puanı orta düzeyde; anksiyete puanı ortalaması 4.9 ± 5.0 olup, anksiyete ölçek puanı hafif düzeyde bulundu. Araştırmamızda ikincil travmatik stres düzeyi; hemşirelerde (p=0.012), geçmişte bir afete maruz kalmış olanlarda (p=0.013), depreme doğrudan maruz kalanlarda (p<0.05), deprem bölgesinde yakını olanlarda (p<0.05) daha yüksek bulundu. Depresyon puanları; deprem bölgesinde yakını olanlarda (p<0.05), deprem nedeniyle bir yakınını kaybedenlerde (p<0.05), deprem sonrası psikososyal destek ihtiyacı hissedenlerde (p<0.05) daha yüksek bulundu. Anksiyete puanları; geçmişte bir afete maruz kalmış olanlarda (p=0.034), depreme doğrudan maruz kalanlarda (p<0.05), deprem bölgesinde yakını olanlarda (p<0.05) daha yüksek bulundu. Kadınlarda (p<0.05), iş yükü artanlarda (p<0.05), deprem nedeniyle bir yakınını kaybedenlerde (p<0.05), deprem sonrası psikososyal destek ihtiyacı hissedenlerde (p<0.05) ikincil travmatik stres, depresyon ve anksiyete düzeyleri istatistikel olarak daha yüksek bulundu. Afet mağdurlarına yardım hizmeti sunan sağlık çalışanlarına, riskli gruplara öncelik verilerek; ruh sağlığını korumaya ve stres ile başa çıkmaya yönelik eğitim, psikososyal destek programlarının geliştirilmesinin önem arz ettiğini düşünmekteyiz. The Kahramanmaras earthquake that occurred on 6 February 2023 in Turkey caused many casualties and injuries. Secondary and tertiary health care services were provided in our hospital to the earthquake victims who were transferred from the earthquake zone and settled in Sivas province. Professional groups working in disaster situations may develop various mental symptoms as a result of their interaction with trauma victims. In this study, it was aimed to evaluate the levels of secondary traumatic stress, depression, and anxiety in health care workers serving earthquake victims in our hospital. In this cross-sectional study, a total of 632 healthcare professionals, including 228 physicians, 74 intern physicians, and 330 nurses, who were determined by systematic sampling method among healthcare professionals working in units serving earthquake victims, were included. Sociodemographic and occupational characteristics of our participants were evaluated with the Secondary Traumatic Stress Scale (STSS) and the Depression, Anxiety, Stress Scale-21 (DASS-21). The mean total score of the secondary traumatic stress scale was 42.5 ± 14.4; the mean depression scale score of the DSAS-21 was 6.7 ± 5.7 and the depression scale score was moderate; the mean anxiety scale score was 4.9 ± 5.0 and the anxiety scale score was mild. In our study, the level of secondary traumatic stress was found to be higher in nurses (p=0.012), those who had been exposed to a disaster in the past (p=0.013), those who were directly exposed to the earthquake (p<0.05), and those who had relatives in the earthquake zone (p<0.05). Depression scores were higher in those who had relatives in the earthquake zone (p<0.05), those who lost a relative due to the earthquake (p<0.05), and those who felt the need for psychosocial support after the earthquake (p<0.05). Anxiety scores were higher in those who had been exposed to a disaster in the past (p=0.034), those who were directly exposed to earthquake (p<0.05), and those who had relatives in the earthquake zone (p<0.05). Secondary traumatic stress, depression and anxiety levels were statistically higher in women (p<0.05), those with increased workload (p<0.05), those who lost a relative due to the earthquake (p<0.05), and those who felt the need for psychosocial support after the earthquake (p<0.05). We think that it is important to develop training and psychosocial support programmes to protect mental health and cope with stress by giving priority to health workers who provide assistance to disaster victims and risky groups.
