Özet
Araştırma, planlanmış davranış modeline dayalı uygulanan müdahale programının kadın mahkûmların sigara içme tutum ve davranışlarına etkisinin belirlenmesi amacıyla yapılmıştır. Araştırma, tek gruplu ön-test, son-test modeline dayalı deneysel türde gerçekleştirilmiştir. Araştırmanın evrenini, İç Anadolu Bölgesi'nde bulunan bir il merkezi sınırları içerisinde yer alan Kadın Açık Ceza İnfaz Kurumu'ndaki kadın mahkûmlar oluşturmuştur. Araştırmanın örneklemine, düzenli sigara kullanan, çalışmanın başlayacağı tarih itibariyle 12 ay içerisinde tahliyesi olmayan ve araştırmaya katılmayı kabul eden 64 kadın mahkûm dâhil edilmiştir. Araştırma, Eylül 2022-Mayıs 2024 tarihleri arasında gerçekleştirilmiş ve kadın mahkûmlara PDT-SBP Aralık 2022-Mayıs 2023 tarihleri arasında altı ay süre ile uygulanmıştır. PDT-SBP'nın içeriğinde sağlık eğitimi, bireysel danışmanlık, grup yürüyüşü, yazılı materyaller (afişler ve broşürler) ile harekete geçiriciler (kumbara) yer almıştır. PDT-SBP tamamlandıktan sonra dördüncü, sekizinci, on ikinci ve yirmi dördüncü haftalarda izlemler gerçekleştirilmiştir. Araştırmada veri taplama aracı olarak; Kişisel Bilgi Formu, Sigara İçme Tutum Ölçeği, Sigarayı Bırakmaya Yönelik Niyet ve Öznel Norm Ölçeği, Fagerström Nikotin Bağımlılık Testi, Sigara Bırakma Durum Tespit Formu ve CO Ölçüm Cihazı kullanılmıştır. Araştırma verileri IBM SPSS V23 ile analiz edilmiştir. Verilerin analizinde tanımlayıcı istatistiksel yöntemlerin (sayı, yüzde, ortalama, standart sapma) yanı sıra t testi, Kruskall Wallis testi, Mann Whitney U testi, Pearson Korelasyon analizi ve spearman korelasyon analizi kullanılmıştır. PDT-SBP tamamlandıktan sonra yapılan son izlemde kadın mahkûmların %29,7'sinin sigarayı bıraktığı görülmüştür. Ayrıca yapılan son izlemde sigarayı bırakan kadın mahkûmlarda Fagerström Nikotin Bağımlılık Testi ve Sigara İçme Tutum Ölçeği puan ortalamaları açısından anlamlı farklılığın olduğu (p<0,05), CO ppm seviyesinde ve günlük sigara miktarında izlemlere göre azalmasının istatistiksel olarak anlamlı olduğu saptanmıştır. Sigara kullanmaya devam eden kadın mahkûmlarda da yapılan izlemlerde Fagerström Nikotin Bağımlılık Testi, Sigara İçme Tutum Ölçeği ve Sigara Bırakmaya Yönelik Niyet ve Öznel Norm Ölçeği puan ortalamaları açısından anlamlı farklılığın olduğu (p<0,05), yine CO ppm seviyesi ve günlük sigara miktarında azalmanın istatistiksel olarak anlamlı olduğu saptanmıştır. Bu bulgular ışığında, ceza infaz kurumlarında sigara kullanımının azaltılması ve sigarayı bırakmaya yönelik etkili programların geliştirilmesi için benzer koşullarda diğer ceza infaz kurumunda bulunan kadın mahkûmlara Planlanmış Davranış Modeline Dayalı Sigara Bırakma Programlarının uygulanması önerilmektedir. The research aimed to determine the effect of the intervention program based on the Planned Behavior Model on smoking attitudes and behaviors of female inmates. The study was conducted in the form of a single-group pre-test, post-test experimental design. The universe of the research consisted of female inmates in the Women's Open Penal Institution located within the boundaries of a provincial center in the Central Anatolia Region. The sample of the study included 64 female inmates who were regular smokers, had not been released within 12 months from the start date of the study, and voluntarily agreed to participate in the research. The research was carried out between September 2022 and May 2024, and the PDT-SBP was implemented for a period of six months from December 2022 to May 2023. The content of PDT-SBP included health education, individual counseling, group walking, written materials (posters and brochures), and incentives (piggy bank). Follow-up assessments were conducted at the fourth, eighth, twelfth, and twenty-fourth weeks after the completion of PDT-SBP. Data collection instruments used in the study included Personal Information Form, Smoking Attitude Scale, Intention and Subjective Norm Scale for Smoking Cessation, Fagerström Nicotine Dependence Test, Smoking Cessation Status Determination Form, and CO Measurement Device. The research data were analyzed using IBM SPSS V23. Descriptive statistical methods (frequency, percentage, mean, standard deviation) as well as t-test, Kruskall Wallis test, Mann Whitney U test, Pearson Correlation analysis, and Spearman correlation analysis were used for data analysis. In the final follow-up assessment conducted the completion of PDT-SBP, it was observed that 29.7% of female inmates had quit smoking. Additionally, in the final follow-up assessment, it was found that there was a significant difference in the average scores of the Fagerström Nicotine Dependence Test and Smoking Attitude Scale among female inmates who quit smoking (p<0.05), and the decrease in CO ppm level and daily cigarette consumption was statistically significant according to the follow-up assessments. In the follow-up assessments conducted for female inmates who continued smoking, it was also determined that there was a significant difference in the average scores of the Fagerström Nicotine Dependence Test, Smoking Attitude Scale, and Intention and Subjective Norm Scale for Smoking Cessation (p<0.05), and again, the decrease in CO ppm level and daily cigarette consumption was statistically significant. In light of these findings, it is recommended to implement Planned Behavior Model-Based Smoking Cessation Programs for female inmates in other correctional facilities under similar conditions to reduce smoking rates and develop effective smoking cessation programs.
