EMLA krem'in peribulber anestezi sırasındaki ağrıya etkisi
Özet
AMAÇ: Peribulber anestezi sırasında ortaya çıkan ağrıyı önlemede emla %5 kremin etkinliğini değerlendirmek. GEREÇ VE YÖNTEM: Fakoemülsifikasyon ile katarakt ekstraksiyonu yapılan 31 hasta rasgele 2 gruba ayrıldı. Birinci gruba peribulber anesteziden 1 saat önce emla krem uygulanan 16 hasta ve ikinci gruba emla krem uygulanmayan 15 hasta alındı. Hastalara hissettiği ağrının şiddeti sorularak, 0-3 arası skorlandı. BULGULAR: Birinci grupta derece 0'da 6 (%37.5), derece 1'de 9 (%56.3), derece 2'de 1 (%6.2) hasta ve derece 3'de ise hasta yoktu ve ortalama ağrı skoru 0.69±0.6 idi. İkinci grupta ise derece 0'da hasta yoktu, derece 1'de 3 (%20), derece 2'de 7 (%46.7), derece 3'de 5 (%33.3) hasta vardı ve ortalama ağrı skoru ise 2.13±0.74 idi. İkinci grupta ağrı skoru istatistiksel olarak daha yüksekti(p=0.008). Emla kreme bağlı herhangi bir yan etki saptanmadı. SONUÇ: Bu sonuçlar peribulber anestezi sırasında enjeksiyona bağlı ağrının azaltılmasında, emla krem uygulamasının güvenli ve etkili olduğunu göstermiştir. PURPOSE: To evaluate the effect of emla 5% cream for preventing the pain during peribulbar anesthesic injection. MATERIAL AND METHODS: The study, included 31 patients who underwent cataract extraction by phacoemulsification. The patients were randomly divided into two groups. We applied emla cream to the first group (16 patients) one hour before peribulber anesthesia and to the second group which consists of 15 patients, we did not use emla cream. The patients were asked for the pain severity and the pain was scored between 0 and 3. RESULTS: In the first group there were 6 patients (37.5%) in grade 0, 9 patients (56.3%) in grade 1, 1 patient (6.2%) in grade 2 and no patient in grade 3 and the mean pain score was 0.69±0.6. In the second group there was no patient in grade 0 and there were 3 patients (20%) in grade 1, 7 patients (46.7%) in grade 2, 5 patients (33.3%) in grade 3 and the mean pain score was 2.13±0.74. The pain score was statistically significantly higher in the second group (p=0.008). Furthermore the side effect whichever related to emla cream was not determined. CONCLUSION: These results had shown that using emla cream for the release of pain related to injection during peribulbar anesthesia was effective and safe.
