Comparison of alfentanil/propofol with fentanyl/propofol for tracheal intubation without muscle relaxants in children [Cocuklarda kas gevseticisiz entubasyon sirasinda alfentanil/propofol ile fentanil/propofol induksiyonunun karsilastirilmasi]
Özet
In order to compare alfentanil and fentanyl in combination with propofol for orotracheal intubation without neuromuscular block in children, 30 patients of ASA I, scheduled for tonsillectomy were included in the study. After premedication with midazolam 0.3 mg/kg oral, patients were divided into two groups (n=15). Alfentanil 20 ?g/kg and fentanyl 2 ?g/kg were given intravenously in group A and group F respectively. All patients received propofol 2.5 mg/kg for anaesthesia inductions. After induction, patients were intubated by the same anaesthesia. Intubating conditions were assessed according to a scoring system. There was no statistically significant difference between groups in intubating score. A significant decrease in systolic arterial pressure (SAP) occurred in all times in group F (p<0.01). In group A only SAP during intubation was significantly lower than baseline (p<0.05). There were no statistically significant differences in SAP between the groups except intubation course (p<0.05). There wasn't any significant difference in heart rate (HR) between groups. During the operation, in four patients of group A (26.6%) and three patients of group F (20%), signs of inadequate anaesthesia were observed. In conclusion, fentanyl in combination with propofol may be a good alternative to the combination of alfentanil and propofol for tracheal intubation without muscle relaxants in children.
